RESOLUTION NO. 14, S-2024

RESOLUTION APPROVING BARANGAY DANGCOL TO ENROLL IN E TAX

PAYMENT SYSTEM OF LANDBANK OF THE PHILIPPINES AND REQUESTING
REPLACEMENT OF LOST CERTICATE OF REGISTRATION ( BIR FORM 2303 )
WITH TIN NUMBER 004-088-599-000.

WHEREAS, BARANGAY DANGCOL wishes to apply for replacement of lost

Certificate of Registration to the Bureau of Internal Revenue (BIR);

NOW, THEREFORE, be it resolved, as it hereby resolved, that, the Sangguniang

Barangay of Dangcol authorize to proceed with its application for the registration with
the BIR; and

RESOLVED, FURTHER, as it hereby resolved, that the BARANGAY DANGCOL

authorized MAYCIEL E. DIMABAYAO, to be the designated representative of the
Sangguniang Barangay of Dangcol in all dealings and transaction with the BIR.

RESOLVED, to authorize BARANGAY DANGCOL with Current Account Number

0442-1014-42 to enroll in the e-Tax Payment System of Land Bank of the
Philippines, and that the authorized users/enrollees of the said system shall be any
two (2) of the following:

2. MAYCIEL E. DIMABAYAO Barangay Treasurer
 RESOLVED FURTHER, that the Land Bank of the Philippines is authorized to
debit the account above mentioned and/or all other accounts upon our instruction in
order to effect our e-Tax Payment.

RESOLVED FURTHERMORE, that this resolution will serve as our intent to enroll
in the Bureau of Internal Revenue (BIR) eFiling and Payment System (BIR eFPS) and
in the Landbank of the Philippines (ETax) and Payment System (BIR eTPS).

RESOLVED FINALLY, that this resolution shall be deemed valid and effective
unless notice of its revocation or amendment is delivered to Land Bank of The
Philippines.

IN WITNESS WHEREOF, I/We hereby sign this document on 29th day of March,
2024 at Barangay Dangcol Balanga City, Bataan.
