1- Purpose
Clear the method of hazard identification, risk
assessment, and risk control;
Then maintain this information continually updated.
2- Scope
This procedure applies to all processes and activities
that have an impact on the safety and health of
employees, which include:
 Routine or non-routine processes or services.
 Processes or services are carried out by company
personnel (Primary or temporary personnel) or
subcontractors or visitors.
 Process and activities related to emergency
preparedness.
3- Responsibilities and authorities
3-1 HSE department:
3-1-1 Identify Hazard and assessment health and
safety hazard and environmental impacts in
consultation with employees and supervisors in the
workplace;
3-1-2 Shall take the role of guidance in conducting
risk identification, risk assessment and identification
of environmental aspects;
3-1-3 Control measures implemented where
appropriate based on the hierarchy of control;
3-1-4 Record maintained of all Risk/Impact
assessments;

Estimate of the severity of losses and damages
resulting from the danger of persons, equipment,
property and the environment; categories may be
catastrophic, critical, serious, minor and negligible).
4-19 Frequency of exposure
The estimate of frequency of exposure to certain risks
when performing the same procedure every time or
once a year or rarely occurs.
4-20 Documented information
Information required to be controlled and
maintained by an organization and the medium on
which it is contained.
4-21 Outsource
Make an arrangement where an external
organization performs part of an organization’s
function or process.
4-22 Incident
Occurrence(s) arising out of or in the course of work
that could or does result in injury and ill health.
4-23 Corrective action
Action to eliminate the cause(s) of a nonconformity
or an incident and to prevent recurrence.

 Natural hazards

 Mechanical hazards
From collisions or contact between the worker's

 Chemical hazards
Caused by the use of chemicals or their leakage into
the working environment such as gases, vapors or
dust. Chemicals include (flammable liquids, corrosive
liquids, oxidants, toxins and carcinogens).
 Negative Hazard
Non-availability of methods of rescue, ambulance and
hygiene, or that organize workplaces and ensure that
workers in places of food and drink receive certificates
indicating their absence from epidemics and
infectious diseases.
 Hazard related to human work
Everything related to the mutual influence between
Human machinery and machinery.
Situations: working at heights, working in confined
spaces, working alone, worker fatigue.
 Biological Hazards
The risk relating to the ill health due to infection by a
virus, bacteria, etc.
