1. PROJECT DESCRIPTIONGENERAL SCOPE
This procedure outlines the general requirements for ……………………………. among

PT. PERTAMINA KILANG INTERNATIONAL (hereinafter referred to as OWNER) and
PT. REKAYASA INDUSTRI, (hereinafter referred to as CONSULTANT) during the
execution of FEED GSH Refinery Unit III Plaju & FEED GSH Refinery Unit VI Balongan
(hereinafter referred to as PROJECT).
This procedure is not meant to circumvent or override any provisions of Contract. In-
case of any conflict between this procedure document and contract, provisions of
contract shall prevail over this document. The scope of this procedure is to define the
minimum requirements for all documents to be delivered by CONSULTANT or
SUBCONSULTANT to OWNER.
PT. Pupuk Kalimantan Timur (“OWNER”) is a subsidiary of PT Pupuk Indonesia
(Persero) as the holding company engaged in the production of ammonia, urea, and
NPK located in Bontang, East Kalimantan, Indonesia. OWNER has operated five
ammonia plants and five urea plants with a total annual production capacity of 2.7 million
tons per year of ammonia and 3.4 million tons per year of urea and 300,000 tons per
year of NPK fertilizer.
In an effort to maintain business sustainability, PT Pupuk Indonesia has assigned
OWNER to develop an ammonia-urea plant located in the Fakfak Regency, West
Papua, which has abundant natural gas reserves. The plant capacity is planned to be
2,500 tons per day of ammonia and 3,500 tons per day of urea.

2. IN ORDER TO PREPARE THE STATED PROJECT, OWNER HAS APPOINTED PT.

REKAYASA INDUSTRI (“CONTRACTOR”) TO CONDUCT VALUE ENGINEERING AND
PREPARE OWNER ESTIMATE PREPARATION FOR FAKFAK AMMONIA-UREA PROJECT.
SCOPE
This procedure prescribes how the documents and drawings (“Documents”) shall be issued and
reviewed in between OwnerOWNER and Consultant ContractorCONTRACTOR during project.

DEFINITIONS
This procedure reflects the following OwnerOWNER’s standard procedures where applicable to
Project. Communication is include "Correspondence" and "Electronic Communication". The term
"Correspondence" in this procedure are include; Mail, Facsimile, Minutes of Meeting, Notes
Discussion, and Document Delivery between OwnerOWNER and
ConsultantContractorCONTRACTOR. "Electronic Communication" include e-mail.

4. APPROVAL AND INFORMATION ITEMS

Approval documents from OwnerOWNER must not reduce or eliminate all of part of the
contractual obligations of ConsultantContractorCONTRACTOR. Project document created by
Consultant ContractorCONTRACTOR includes engineering documents intended for approval or
information along with the numbers of required copies. The document should be classified based
on the principles below:
Approval Item (A)
Only documents that are fundamental and are used to further the work process or which are
considered important by OwnerOWNER must be maintained, classed as "Approval Item".
Documents classified as "Approval Item" tailored to the needs of OwnerOWNER mentioned in
Deliverable List of Project Procedure. Any significant deviation, or a significant change, the
document that has been approved intended for the approval of OwnerOWNER.

The documents produced by Consultant ContractorCONTRACTOR are categorized as

"Information Document" means a document submitted to OwnerOWNER not to request approval
from OwnerOWNER. But OwnerOWNER entitled to minor comment on the document.

5. DOCUMENT SUBMISSION, REVIEW, APPROVAL AND REVISION PROCESS
7.1 Process Flow

In order to process the submission, review and revision in efficient and timely anner,
Document shall be completed at 2nd submission to OwnerOWNER as IFA, incorporating

OwnerOWNER’s comment to IFR and official / unofficial correspondence between

OwnerOWNER and ConsultantContractorCONTRACTOR.
Document submission, review and revision process is summarized in Attachment-1. Further
detailed procedures are described in the following sections of this document.
All Documents shall be handled in accordance with Project Execution Plan - FIOR-PJ-PRO-
0004, and MOM Kick Of Meeting dated April 3rd, 2023.
Document Approval
Comments shall be directly made on the Document, and reasonably clear and specific so
that the document ownerOWNER can easily understand for revision. The reviewing party
shall avoid the following comments:
Inconsistent comments (in case of multiple reviewers)
Conflict comments with the previous comment
Questioning comments
Results of the Document review by OwnerOWNER or Consultant ContractorCONTRACTOR
shall be categorized in the following status :
Approved - A.
Consultant ContractorCONTRACTOR can process to the next step.
Approved with Comments - B.
Documents can be processed by incorporating all the comments or selected comments as
agreed with CompanyOwnerOWNER
Major Comment - C
Documents need to be further discussed with OwnerOWNER. Consultant
ContractorCONTRACTOR can not issue revision documents before OwnerOWNER and
Consultant ContractorCONTRACTOR have agreed abour the concern.

The result from discussion with OwnerOWNER is noted in Minutes of Meeting, for further
document status shall be rises to the next step. The discussion shall be conducted not later
than 2 working days after receiving the comments from Company.Document have significant
deficiencies and errors to the extent that a complete rework is required. Alignment meeting
among OWNER and CONTRACTOR shall be held to rectifying all of document issues.
Document to be re-submitted to OWNER with the same status. Work may not be proceeded
until the document has been resubmitted, reviewed, and returned as Code A, B or C.
7.2 Revision Track

All revision from the previous submission to OwnerOWNER shall be clearly identified with
cloud mark, revision track (for MS Word) or color (for MS Excel), and at the Revision History
Sheet.
Identification and Status Stamp

Identification of the document status will be provided to write in transmittal document sent to
OWNEROWNER. This information can also be shown on the first page of each document in
the box adjacent to the title or letter head. The documents with the Issued for Review, Issued
for Approval & Issued for Information status that have been approved will give signature as
attached sample.

OwnerOWNER document control will send respond (comment or approval) through transmittal to
ConsultantContactor with statement 'approved or approved with minor comments or revised as
comments’ within time period as described below:

Time period for Owner ClientOwnerOWNER review on each revision of Issued for Review (IFR)
or Issued for Approval (IFA) is 3 working days. If one day the owner has not issued a comment
for 5 working days, the contractor is allowed to send further revisions.
• ConsultantContactor should shall official remind the OwnerOWNER before the due
date.
• In the event that ConsultantContactor has not receive any feedback from Company
OwnerOWNER within 5 working days, ConsultantContactor is allowed to continue
process the document to next revision, and the previous document will be dimmed as
approved/ no comments.
• Comments shall be one time for specific subject. In the event that ConsultantContactor
cannot meet with Client’s OwnerOWNER expectation for comment closure, Client
OwnerOWNER shall invite ConsultantContactor to conduct joint review meeting (JRM)
to deliver the Client’s OwnerOWNER expectation and agreed with ConsultantContactor.
After the joint review meeting (JRM), Client OwnerOWNER shall return the document to
ConsultantContactor with “Approved with comments” with comments that mentioned
inside the MOM of joint review meeting (JRM).
• Issued for Approval comments only allow 1 time (Maximum until A1)

• After receiving the opinion of the OwnerClientOwnerOWNER, ConsultantContactor must
take the following actions:
• Review documents to incorporate Owner ClientOwnerOWNER comments and to
describe the result of clarification
• Conduct Meeting with ClientOwnerOWNER as described in point (2) and (3)
belowabove, and re-issued these documents for signature stamp and then the document
should be in re-issue "For Bid" after "Approved" signed by OwnerClientOwnerOWNER.
• Send notification to Communicate with the Owner ClientOwnerOWNER for comment
