Sir/Madam:

Attached is my affidavit regarding my failure to submit certification of my


employment with the Provincial Government of Nueva Vizcaya from July 1 to
December, 1976. I went to the Human Resource Management Office of the PLGU
but they cannot locate my 201 file in their records. I went also to the Provincial
General Service Office and they were able to issue me a Xerox copy of the renewal
of my appointment dated July 1, 1977 onwards. It was disclosed by them that
such records is no longer with them. I tried the local Civil Service Office but to no
avail. I was advised to try the Regional Civil Service Office but just the same, they
do not have my record pertaining to that date.

I attached hereof a photocopy of my appointment with the Provincial


Government dated July 1, 1977 and please take note of PANSAMANTALA
(RENEWAL]. Could it mean that there was something issued previously to be
renewed?

Sir/Madam, I plead for your loving and merciful consideration on the matter. I
have waited a very long time for my retirement pay and I am in utter poverty right
now as my creditors come to me left and right with their collection demands .Very
stressful! Can I enjoy yet my retirement pay? Please help! I am in dire need. . . . .
