A 60-year-old female patient underwent a CT scan of the head which found a massive right basal ganglionic brain bleed measuring 70x50mm that extended into the ventricles, compressing them and causing a 16mm midline shift to the left. The cerebellum and brainstem appeared normal, but blood was observed in the fourth ventricle. The impression was of a massive right basal ganglionic bleed with intraventricular extension and mass effect.
