Documentation: Signed copy of Cross section drg. for flooring


& BBS were not still available at site during the time of

Inspection. Also MTC of cement & Design Mix (M 30
grade) still not submitted from vendor.

Boundary wall of brick work was going on dry mortar


based instead of wet mortar with Cement Sand (1:6)
ratio, it was suggested that the ratio must be (1:4) in
wet condition based as per standard specification wise.

Also before start the brick work, water sprinkling reqd.
on bricks & Curing reqd. min 7 days basis as per IS
2212 wise. Plastering reqd. in both side of brick wall as
per standard specification wise.

Fly Ash Brick Sample testing reqd. as per IS 12894 &


3495 Part (Part 1 to Part 4) for two types of brand asap IMMEDIATE Major Open
as per QAP wise.

Before fixing the TMT Rebar, rusting cleaning by wire


brushes must be reqd. asap, also same had been IMMEDIATE Medium Closed
suggested at site during the time of Inspection.

Documentation: Calibration report of Ajax & BBS (Excel IMMEDIATE Medium Open
format) were not still available or submitted by vendor.

Curing with water ponding & hessian cloths reqd. asap.

checked as per design mix (M 30 grade) & found that

MTC of Rebar (Jindal steel) & calibration report of
Ajax (for RCC panel purpose) still not provided by

presence of VL personnel.
During the time of casting of IPS (the avg. thickness
found 25 mm to 40 mm), panel size found same as
RCC panel (5.8 m* 5.8 m), but according to IS 2571
(clause 5.7) wise it should not be more than 4 m in both

way. Henceforth, it is strongly recommended that, the
panel size should be maintained as per IS 2571 wise.
Apart from that, the ratio of mix preparation for IPS;
should be followed as per IS 2571 (Table 3) wise.

Curing (for end side panels DOC 03.10.19) was not

MTC of Sika (Chapdur) for used as hardener (over IPS


panels) reqd. asap & used as per mentioned the
methodology as per mfg. guideline wise. The IPS
materials as per Table 3 wise, the higher size of Agg.
12.5 mm graded should be used with Zone II sand IMMEDIATE Major Open
particle, but at site it was not followed. The Gradation
report of Agg. (12.5 mm graded) & Sand (Zone II) as
per IS 383-2016 wise reqd. asap. Also RCC panels top
level was not taken before start of IPS Job.

Documentation: As per FIR No- 167 wise, still dated IMMEDIATE Major Open
documents were not submitted by vendor.

Finished IPS panel (DOC-16.10.19) found self-


damaged on dated 19.10.19 as per site observation
wise; due to that, good wearing resistance & surface
hardener were not done as per site requirement wise.
Also process of spraying the dry-shake was not
IMMEDIATE Major Open
followed as per Sika (Chapdur) methodology wise,
after removing the surface bleed water or allow to be
evaporated, Chapdur materials had been applied in two
stages. Also final finishing was not done by repeating
several time of hand Trowel.

The IPS finished floor had been damaged due to above


mentioned methodology (as per mfg. guideline wise);
was not followed properly. Henceforth, rectification
reqd. asap. by dismantling this effected area of IPS.
However, the preparation of IPS by using higher size of
IMMEDIATE Medium Open
Agg. 12.5 mm (Graded) with Zone II sand as per IS
2571 (Table 3) wise; it will be (1:1:2) as per Sika
(Chapdur) guideline wise. Gradation test reqd. of (12.5
mm) graded agg. & Sand (Zone II) as per random
sample collection from site before starting the Job.
DOC (27.09.19) for 03 No RCC panels; after 28 days
curing M 30 grade of concrete, cube test had been
checked on surface dry condition based & found 29.48

N/mm2 & 29.33 N/mm2 (out of 12 No cubes), while
reqd. min 30 N/mm2 as per IS 456 & IS 516 wise on
dated 25.10.19. Hence unsatisfactory.

DOC (03.10.19) for 04 No RCC panels; after 28 days


curing M 30 grade of concrete, cube test had been
checked on surface dry condition based & found
compressive strength less than 30 N/mm2 on dated
31.10.19, while reqd. min 30 N/mm2 as per IS 456 & IMMEDIATE Major Open
IS 516 wise. Hence unsatisfactory. Also 02 No panel cubes
had not been taken from site during the time of casting. It is
suggested that, the failed panels core test should be conducted as
per IS 516 part 4 - 2018 wise asap.
