II. FAMILY COMPOSITION


Roberto S. Andres : 58 years old, College, government employee
Ger- B F. Andres : 33 years old, single, college graduate, cooking,
son;
Robert Rodsher F. Andres : 21 years old, single, college graduate, son.

III. PROBLEM PRESENTED


Client husband prompted to seek possible social case study, for him to ask financial assistance
from the Philippine Charity Sweepstakes Office (PCSO), DSWD Mimaropa and other entities who
help those patients who needs medical assistance.

IV. FINDINGS
Chuchi F. Andres is 60 years old a bonafide residence of Poblacion, San Fernando, Romblon.
She is at present afflicted of kidney chronic disease stage 5 which is too detrimental to human health.
At present she is jobless due to her ailment. Her present ill health condition made the whole family
to suffer economically where some basic needs is sometimes deprived just to sustain the prescribed
medicines amounting to more or less Eighteen Thousand Pesos(P18, 000.00). Such stated amount
aggravates the economic condition Eighteen Thousand Pesos a month allotted to the medicines alone
is already a big deal as to basic needs is concerned. To be afflicted with kidney chronic diseases
stage 5, is emotionally, psychologically and financially is really a great burden. The prescribed
eleven (11) kinds of medicines since 2016 to 2018 amounting to more or less eighteen (18) thousand
pesos monthly really incapacitate the whole family in many ways. Her husband monthly net income
is not enough for the monthly allotted budget alone for the prescribed medicine. Further the regular
schedule of check-up of the patient since 2016 to October 2018 really triggers economic constraints.
Such stressful condition made the immediate family, financially and socially disadvantaged as the
patient has to continue the long term treatment because of Chronic Diseases Stage 5.
However, as disclosed by the husband of the patient since 2016 to October 2018, almost or
more than half a million was incurred to include laboratories and medicines. Unluckily, last October,
2018 the result of the scheduled check-up is a two times a week dialysis was highly recommended as
the ultimate medical intervention by her attending doctor. Now, no option but to adhere the doctor’s
advice. So since October, 2018 to date, the patient submitted for two times a week sessions of
dialysis. Bringing the patient to the dialysis center is very difficult, because, the patient could hardly
walk because of her disability. As a physically disabled and at the same time afflicted by the Kidney
Chronic disease, proper treatment, love and care should be well established among family members
so that the patient will be inspired.
Health is really wealth. At present limited financial resources is a problem as the course of
treatment of two times dialysis is lifetime. Both internal and external resources was maximized yet
the amount accumulated is not enough.

2. Made Social Case Study Report;
3. Provided Counseling

VI. EVALUATION AND RECOMMENDATION


Based from the above findings and justification, the above mentioned patient is eligible financial
assistance. Thereby, the family of the patient is really incapacitated to finance the needed medical
intervention the two times a week dialysis.
In like manner proper counseling, was highly recommended.
