MONITORING AND EVALUATION FORM

To secure a clean, mosquito-free and conducive environment for the pupils and teachers, and
as mandated in the memorandum issued by the DepEd Division of Isabela in support to the project of
our Provincial Government “Todas Dengue, Todas Na To, Ika-pitong Kagat”, the San Miguel
Elementary School held its annual Clean-Up drive last June 12, 2020 in preparation for the School
Year 2020-2021.
The Clean-up Drive is designed to bring together multi-sectoral taskforce to work together in
promoting a clean environment and implementing preventive measures to keep the pupils , faculty
and staff from mosquito-related diseases. With this in mind, the administration of the SMES
contacted individuals and organizations , public service and concerned citizens. At least 74
volunteers participated in the 2020 Clean-Up Drive. Among the activities done were cleaning the
back areas of classrooms, clearing drainages, and disposing water from cans, tires and drums.
The volunteers gathered in the SMES early morning of June12 to start the real work. Soon,
mosquito habitat such as drums and bottles were disposed. The teachers and other volunteers also
checked the rooms and other areas to see whether the school is ready for the opening of classes.
The said program truly signifies the importance of public-private partnership in creating a
better environment for the pupils.
