Work Completed - Supply and installation of two fiberglass storage tanks whose
capacities are 10 m3 each
- Pipeline work
- Rehabilitation of existing cattle trough
- Installation of new generator set provided by the Woreda (plan was
to replace generator starter key)
- Tying the storage tanks firmly by 6mm diameter reinforcement bar
with J-bolts on the existing concrete base slab

The scheme is not providing service to the community due to


corroded and damaged main distribution pipe near the elevated
tank.

- Construction of steel structure to increase elevated tank slab size
- Pavement construction around the elevated tank stand

- Connecting inlet and outlet pipes of the elevated reservoir

feed the surface pump

- Construction of 9.50 m high RCC water tank tower
- Installation of 50 m3 pioneer steel tank on elevated seat.

Currently, the system provides service to the community by the


existing structures.

Recommendations Maintain the crack by patching, re-plastering, or other appropriate


methods and sufficiently cure concrete/mortar to avoid such cracks.
Quality Management (2)
New Issue Identified Lean concrete placed for the box culvert at chainage 0+131 m has
silted-up by the overburden soil. Further, the working area round the
structure is limited, restricting easy movement of workers (this culvert
was not in the original scope of work and it was proposed under the
change requested for this site).

New Issue Identified Vertical wall reinforcement bars are placed at a spacing of ~30 cm
apart at some places. The design document indicates to have a spacing
of 20 cm between reinforcement bars.

Recommendations It is better to provide and assemble reinforcement bars as indicated in


the design drawing.
Quality Management (2)
New Issue Identified Installed ventilation louvers are not according to design, the louver
slots are arranged vertically, far apart from each other and could allow
rain to enter into the building. Also, provided insect screen is simply
tied with wire on the ventilation louver slots.
