5.4. Pilot Ladder


Pilot ladder installed on offshore platform/vessel is used only for
emergency condition/evacuation.
Requirements below are applied for pilot ladder installed at third party’s
tankers during off-take.

5.4.1. Add Pre-use requirements

(gangway) must be used in conjunction with pilot ladder.

backpack). Ship's crew should drop a line down to the boat and pull it up.
h) Always maintain 3 point contact during ascending/descending
and practice the art of climbing a pilot ladder.

1 Purpose
To ensure that all transfers of personnel from a platform to a vessel and vice-versa is
done safely and in line with COPI safety expectations regardless of the method used.

2 Application
All COPI offshore facilities.

4 Definitions
Marine Personnel Transfer Capsule: A piece of lifting equipment specially designed for
the safe transfer of personnel between a platform and a boat and vice-versa using the
platform crane as the lifting device. Commonly called a FROG Personnel Transfer
Capsule within COPI.
Swing rope: a rope which is attached to a rigid structure just above water level such that
personnel are able to use it to swing from the platform to the supply vessel and vice-
versa.
Personnel basket: a piece of lifting apparatus designed to transfer personnel over water
from a platform to a vessel and vice-versa using the platform crane as the lifting device.
Gangway: A ramp used as a removable footway between a vessel and a pier or platform.
Pilot ladder: a hanging ladder of ropes or chains supporting wooden or metal rungs or
steps. Also known as a Jacob's ladder or a jack ladder.

5 Responsibilities
VP Operations is responsible for the creation of the policy and procedure for personnel
transfer and lifting within COPI. The Offshore Operations Senior Managers, or their
delegates, are accountable for the implementation of the policy and procedures within
their management remit.

6 Procedures
Personnel Transfer Mode: It shall be COPI policy that the first choice of personnel
transfer method will be by FROG Personnel Transfer Capsule. Any deviation from this
requirement requires approval from management through HSE exception.

6.1 FROG Marine Personnel Transfer Capsule


5.1.1 Weather:
• Maximum swell: 5-7 feet.
• Maximum wind speed: Offshore Crane Limitation.
5.1.2 Personnel Condition:
• Physically Healthy. Person in charge of transfer to confirm with the transferring
personnel.
• Have undergone Marine Personnel Transfer Capsule training.
• Any transfer by Marine Personnel Transfer Capsule shall be at the individual
transferring discretion. Person in charge of transfer to ask the person if he/she is
confident or not to do the transfer.

5.1.3 Inspection
• Prior to use: Person in charge of transfer is to visually check condition of the FROG
Marine Personnel Transfer Capsule (as per check list):

o The FROG is fully certified and its data plate is stamped with a visual
inspection date not more than 6 months ago and a load test date not more
than 12 months ago.
o The sling is in good condition, no damage, not kinked/twisted.
o The safety latch works well.
o It must be equipped with two tag lines with minimum length of 10 feet,
without knots or frayed ends.
o Each leg of the lifting sling is correctly attached to the appropriate lifting eye
via screw pin shackles with spilt pins in place.
o Check that two M16 bolts, nuts and wire seals that secure the lifting points to
the Main Support Tube are properly fitted.
o Check the Back-up Lifting eyebolt, nut and wire seal are fitted correctly and
in good order.
o Check all Seat Harnesses to ensure that the release buckles operate
properly. Check that all Seat Harness attachment points are secure.
o Check that the roll pin in the Keel Plate Nut is in position.
o Check that all capsule fittings, framework and buoyancy are in good order.
• Semi annual and annual inspections shall be carried out as per manufacture
guidance.
• Semi annual inspection to include sling inspection.

5.1.4 Departing and Landing Platform


Transfer Landing Area Condition:
• Minimum free space for FROG Capsule landing on platforms is 8 feet radius and
on the boat is 10 feet radius, under discretion of Crane Operator, with additional
area for personnel access.
• Adequate lighting for crane operator to conduct the transfer safely must be
provided.
• Landing areas must be free of slippery materials and trip hazards.
5.1.5 Marine Personnel Transfer Support
• Boat Captain:
o Has the right to reject or accept the FROG capsule transfer.
o Is to give signal when the transfer can commence.
o Is to ensure that the assigned Boat Crew has been trained for Personnel
Transfer using FROG capsule.
• Boat Crew:
o To wear required PPE, work vest and hand free radio.
o To communicate with person in charge of transfer, Boat Captain, and Crane
Operator.
o Minimum of 2 Boat Crew personnel to assist during transfer.
• Person in charge of transfer
o To wear required PPE, work vest and hand free radio.
o Be readily identifiable as the person in charge of the transfer.
o To communicate with Boat Crew, Boat Captain and Crane Operator.
o Has been trained for Personnel Transfer using capsule and has at least 6
months experience on offshore assignment.
• Escort:
Visitors shall be accompanied by experienced personnel during field tour.

5.1.6 Decision:
• Captain and Person in charge of transfer:
o To decide on use of FROG Marine Personnel Transfer Capsule based on
conditions.
o To decide to which side of the offshore facility to use for transfer.
• Site Line Management approval:

o Accountable in ultimate decision on the mode of Personnel Transfer.


• Individual Discretion:
o Personnel have the right to abort the Personnel Transfer using FROG Capsule.
5.1.7 Execution:
• Personnel to transfer:
o Wear required PPE (including work vest). The helmet has to be strapped to
the chin.
o Hands must be free of “carry items” during the transfer with no back packs.
o Both hands should be used to grip the hand holds on the FROG during the
transfer.
o If rain coat is used, wear it properly, must be buttoned up.
• Propre Marine Personnel Transfer Capsule Techniques:
o Pick up
o Queue in an orderly manner and take it in turn to board the FROG Marine
Personnel Transfer Capsule.
o Do not obstruct the boat captain’s view.
o Ensure all passengers are wearing work vest. Ensure passenger properly
secured by their seat harnesses (and adjust seat harness straps as required).
o Note: A stretcher may be carried by removing seat – refer to instructions
posted in capsule.
o Ensure that any luggage in the capsule is well secured, up to a maximum of 25
kg/passenger.
o Check that the tag lines and lifting tackle is free of any snagging hazards;
check that the lift path is clear of any obstructions.
o Confirm that the passengers are ready for lift off then signal to the crane
operator to pick up slowly.
o Crane operator is to swing the FROG Capsule immediately after clearance of
obstacles to keep it physically minimum time over the boat/platform.
• On Landing
o Guide the FROG Capsule into a suitable landing position using tagline as
required.
o Upon touchdown, the crane operator should immediately lower the hook about
another 10 feet, note: this will create slack in the lightweight tackle and reduce
the risk of snatching of the FROG Capsule due to vessel heave or loss of
station. Slew over to place the hook downwind/weather from the FROG
Capsule to increase the allowable vessel drift.
o Signal to the passengers to release their harnesses and step clear of the
FROG Capsule.
o Do not jump before FROG Capsule lands properly.

6.2 Swing Rope Procedure


Note – Boat must have a recognized swing rope deck landing area before a swing
rope transfer can be authorized.
5.2.1. Weather
• Maximum swell: 3 - 4 feet
• Maximum wind speed: 15 knots
5.2.2 Personnel Requirement
• Physically Healthy. Person in charge of transfer to confirm with the personnel on
his/her condition.
• Have undergone swing rope training.
• Any transfer by swing rope shall be at individual discretion. Person in charge of
transfer to ask the personnel if she/he is confident or not to do swing rope transfer.
5.2.3 Swing Rope Inspection
• Ensure that regular check has been conducted by checking the swing rope color
code.

• Person in charge of transfer to visually check condition of the swing rope.


• The rope is clean, free of oil/grease.
• The rope fitting is in good condition.
• The rope strand has no damage.
• Departing and Landing Platform
• Gap
o Elevation gap: maximum 1 feet 6 inches.
o Horizontal gap: maximum 2 feet.
• Transfer Landing Condition:
o Free space 5 feet width x 3 feet depth minimum.
o Adequate Lighting.
o Free of slippery materials and trip hazards.
• Perform personnel transfer to boat at designated area.
o Rope shall be made of manila fibre with diameter between 1”- 1 ½”.
5.2.4 Personnel Transfer Support
• Boat Captain:
o Has the right to reject or accept the personnel transfer using swing rope.
o To give signal when the transfer can commence.
o To ensure that the assigned Boat Crew has been trained for swing Rope
personnel transfer operation.
• Boat Crew:
o To wear required PPE, work vest and retractable Safety Line.
o To communicate with Dispatcher and Boat Captain using hands free radio.
o Minimum of 2 Boat Crew to assist personnel during transfer.
• Person in charge of transfer
o To wear required PPE, work vest and retractable Safety Line, which shall be
attached to a fixed structure.
o Be identifiable as the person in charge of the transfer.
o To communicate with Boat Crew and Boat Captain using hands free radio.
o Has been trained for swing rope personnel transfer and has 6 months
experience on offshore assignment.
o Person in charge of transfer has to be accompanied with 1 other crew member
to assist personnel during transfer.
• Escort:
o Visitors shall be accompanied by experienced personnel during field tour.
5.2.5 Decision
• Captain and Person in charge of transfer:
o To decide mode of personnel transfer (swing rope) based on weather condition.
o To decide to which side of facility to land.
• Site Line Management approval:
o Accountable for ultimate decision on the mode of personnel transfer.
• Individual Discretion:
o Personnel have the right to abort the swing rope transfer.
5.2.6 Execution
• Personnel to transfer:
o Wear required PPE (including work vest). The helmet has to be strapped to the
chin.
o Hands free of “carry items” during the swing and no back packs.
o If rain coat is used, wear it properly, must be buttoned up.

• Proper Swinging Techniques


o There are two ropes on the landing boat, use the farther one to swing (inboard:
from boat to platform; and outboard: from platform to boat).
o Time the swing in synchronization with boat motion/wave action.
o Grip rope with both hands high enough to adjust to boat movement/relative
elevation gap with the boat landing.

o Use the rope to support full body weight rather than stepping onto boat/landing.
o Do not let the rope get between legs, as it might interfere during landing.
o Do not step on boat bumper.
o Do not release the rope until you completely reach the landing platform. If your
timing is off and you miss the landing platform, do not let go off the rope, continue
your swing and you will be assisted by the dispatchers or landing crews to land
safely.

5.3 Personnel basket (Billy Pugh) Procedure


5.3.1 Weather
• Maximum swell: 5-7 feet
• Maximum wind speed: 20 knots
5.3.2 Personnel Condition

• Physically healthy. Person in charge of transfer to confirm with the personnel on


his/her condition.
• Have undergone personnel basket training.
• Any transfer by Billy Pugh shall be at individual discretion. Person in charge of
transfer to ask personnel if she/he is confident or not to do the transfer.
5.3.3 Personnel Basket Inspection

• Person in charge of transfer is to visually check condition of the Billy Pugh.


o The sling is in good condition, no damage/kinked/loose thread.
o The safety latch works well.
o It must be equipped with two tag lines with minimum length of 10 feet, without
knots or frayed ends.
• The inspection is conducted every 6 months together with the sling inspection.
• The lifetime is 2 years.
5.3.4 Departing and Landing Platform
Transfer Landing Condition:
• Minimum free space for basket landing on boats/platforms is 6 feet radius, under
discretion of Crane Operator, with additional area for personnel access.
• Adequate Lighting for crane operator to conduct the transfer safely.
• Free of slippery materials and trip hazards.

5.3.5 Personnel Transfer Support


• Boat Captain:
o Has the right to reject or accept the personnel transfer using Billy Pugh.
o To give signal when the transfer can commence.
o To ensure that the assigned Boat Crew has been trained for Billy Pugh
personnel transfer.
• Boat Crew:
o To wear required PPE, work vest and hand free radio.
o To communicate with Person in charge of transfer, Boat Captain, and Crane
Operator.
o Minimum of 2 Boat Crew members to assist personnel during transfer.
• Person in charge of transfer
o To wear required PPE, work vest and hand free radio.
o Be identifiable as the person in charge of the transfer.
o To communicate with Boat Crew, Boat Captain and Crane Operator.
o Has been trained for Billy Pugh personnel transfer and has 6 months
experience on offshore assignment.

• Escort:

Visitors shall be accompanied by experienced personnel during field tour.

5.3.6 Decision
• Captain and Dispatcher:
o To decide mode of personnel transfer, Billy Pugh, based on conditions.
o To decide to which side of facility to land.

• Site Line Management approval:


o Accountable in ultimate decision on the mode of personnel transfer.

• Individual Discretion:
o Personnel have the right to abort the Personnel Basket transfer.
5.3.7 Execution
• Personnel to transfer:
o Wear required PPE (including work vest). The helmet has to be strapped to the chin.
o Hands free of “carry items” during the swing with no back packs.
o If rain coat is used, wear it properly, must be buttoned up.
• Proper Personnel Transfer Techniques:
o Queue and take turn boarding the Billy Pugh.
o Do not obstruct the boat captain’s view.
o Maximum of 4 personnel allowed at one time, avoid single person transfer where
possible (especially for visitor).
o Wear a work vest and fasten it securely.
o Do not linger when boarding from a vessel.
o Stand on outer rim of basket and face inwards.
o Grip firmly on the net ropes with both arms through the rope netting, cross arms
and hold firm.
o Keep feet apart and flex your knees.
o Be prepared for sharp jerks during lift off.
o Do not jump before Billy Pugh is landed properly.
o Cushion the landing by flexing your knees.
o On landing, unlock arms, step off and move away from Billy Pugh.
o Crane operator is to swing the personnel basket immediately after clearance of
obstacles to keep it physically minimum time over the boat/platform.
5.4 Gangway
5.4.1 Requirement of Gangway
• Material: aluminum or steel, non slip footway.
• Equipped with handrail (3 feet height, diameter 1 ¼”) and mid-rails on both sides.
• One end of the gangway is equipped with roller.
• Regularly inspected and maintained.
• Not less than 20 inches wide walking surface.
• Electrical grounding is required to connect departing and landing platforms.
• Gangway shall be supported by approved engineering design.
• Gangway has to be certified by marine class inspectorate.
5.4.2 Weather
• Maximum swell: 3 - 4 feet
• Maximum wind speed: 15 knots
5.4.3 Personnel Condition
Physically healthy. Person in charge of the transfer to confirm with the personnel on
his/her condition.
Gangway Inspection
o Ensure checking has been conducted by checking the inspection tag.

o During crew change, Person in charge of transfer to visually check condition of


the gangway. It must be clean, free of slip/trip/fall hazards, i.e. oil, grease, debris,
etc.
5.4.4 Transfer Conditions
• Maximum slope - 15 degrees.
• Adequate Lighting for its full length.
• Free of slippery materials and trip hazards.
• Must be kept clear of obstruction.
• Safety net on both sides.
• Life ring shall be available within the area of transfer.

5.4.5 Personnel Transfer Support (applicable only for boats)

• Boat Captain:
o Has the right to reject or accept the personnel transfer using Gangway.
o To give signal when the transfer can commence.
o To ensure that the assigned Boat Crew has been trained for Gangway personnel
transfer operation.
• Boat Crew:
o To wear required PPE and work vest.
o Minimum of 1 Boat Crews to assist personnel during transfer.
• Person in charge of transfer
o To wear required PPE and work vest.
o To be identifiable as person in charge of transfer.
o To communicate with Boat Crew and Boat Captain using hand free radio.
o Has been trained for Gangway personnel transfer and has 6 months field
experience.
• Escort:
Visitors shall be accompanied by experienced personnel during field tour.
5.4.6 Decision

o To decide mode of personnel transfer (Gangway) based on weather condition.
o To decide to which side of facility to land.
• Site Line Management approval:
o Accountable in ultimate decision on the mode of personnel transfer.
5.4.7 Execution
• Wear required PPE (including work vest).
• If rain coat is used, wear it properly must be buttoned up.
• Hold handrails.
Note: Work vest is not required for personnel transfer on Jetty.
5.5 Pilot Ladder
Pilot ladder installed on offshore platform/vessel is used only for emergency
condition/evacuation.
Requirements below are applied for pilot ladder installed at third party’s tankers during
off-take.
5.5.1 Pre-Use Requirements of Pilot Ladder:
• Wind speed < 20 knots and sea state max. 6 feet.
• Pilot ladder in use must be built and rigged to SOLAS compliant (Chapter V,
Regulation 23 and IMO Resolution A.889 (21)). Pre-use inspection must be
conducted by COPI reps.
• Any transfer by Pilot Ladder shall require vessel’s Master authorization, and to be
attended by officer and a deck party all the times.
• The rigged pilot ladder must be on the leeward side of the vessel.

• Boat operator while alongside the pilot ladder has to maneuver the boat properly
and also be extremely aware of the safety of the boarding party.

5.5.2 Requirements of Pilot Ladder:


• Material: manila rope and wood.
• Regularly inspected and maintained.
• The width of ladder is 1 feet 6 inch – 2 feet.
• Distance between step ladder is 10 inch – 14 inch.
• Use cylinder wood for hanging ladder, diameter 1-1/2 inch.
• Use 1-1/2 inch by 4 inch blocked wood/square for wall-mounted ladder.
• One end of the ladder shall be tied up with pad eye.
6 Pilot Ladder Inspection
• Conduct periodic inspection and put tag upon completion.
• Free of oil/grease.
• When not in use, fold and cover with protective material.
6.1 Execution:
• Standard PPE with additional work vest.
• Stay calm and take your turn.
• Hands free of “carry items” during the transfer and no back packs.
• If the freeboard is 9 meters or more, an accommodation ladder (gangway) must be
used in conjunction with pilot ladder.
• Pilot must be trained or experienced personnel.
• Pilot must wear standard PPE with additional work vest.
• Always maintain 3 point contact during ascending/descending and practice the art
