ACTIVITY 1
Before you begin to prepare building your Portfolio, you need to make sure that you have completed
all the outputs for this course. Go over the list found in LDM2 Guide for Expected Outputs and find out
whether you have completed all the outputs required for Modules 1-4 of this course.
Submit the completed outputs to your designated Coach for final evaluation. Keep a copy of all your outputs
for your own records.

ACTIVITY 2
Study the Reading on Portfolios from the Glossary of Education Reform. Based on your
reading, reflect on the following questions and jot down your answers.
1. How will your Portfolio help you in tracking the progress of your teaching practice in
your School’s LDM?
The Portfolio we made will help us in tracking the progress of teaching practices
by means of:
Evaluating and assesing the quality of coursework quality, learning progress,
and academic achievement.
Deciding whether we have met learning standards or other requirements for
LDM2 course for Teacher.
Assisting us in reflecting goals and progress as learners.
Creating a lasting archive of LDM2 Course for Teachers work products,
accomplishments and other documentation.
will serve as evidences of my teaching and it will clearly show my
improvements as well as the problems I will meet and it will guide me to keep
on the right track.
2. What evidence from the previous modules will help capture the progress of your
teaching practice?
The Individual Development Plan is one of the evidences from the previous
modules that will help capture the progress of my teaching practice. It is a
professional development tool used to help us to identify our strengths and areas
for growth and set attainable goals. The IDP process is a useful way to have
intentional conversations with your mentor/advisor about career interests, skill
building, and planning for your future throughout your time as a Teacher.

Familiarize yourself with the List of Suggested Evidence Form. Is the list of evidence enough to
capture the progress of your teaching practice? What other evidence can you think of that is relevant to the
LDM adopted by your School?

In your LAC Session, discuss your answers and thoughts on the questions from Activities 2 and 3
with your colleagues.
During our discussion we can say that reflection plays an integral part in a
portfolio because it serves as our summary in everything that we do.

ACTIVITY 1
Study the criteria and indicators found in the Evaluation Rubric. What steps will you take to ensure
that you are able to hit the criteria in the Rubric? Write down your answers and any other thoughts about the
Rubric.
To make my portfolio effective, I must follow or align the things that I do based on the given rubric
in order to hit the necessary things needed in making my portfolio.
ACTIVITY 2
Discuss and share your thoughts on the Rubric in your LAC Session. Present your steps to make your
outputs and Portfolio responsive to the evaluation criteria and indicators. Take note of your colleagues’ other
insights as well.
During the discussion with my group we come up with the different ideas on how to make our
Portfolio.
First, we must follow the criteria and give what is needed.
Second, keep and attach all the outputs needed. (make sure to jot down all the notes)
Always put reflection for this will summarize all the things we make in our portfolio.
It should always be organized and pass ahead of time.
REFLECTION
Portfolio is a compilation of academic work and other forms of educational evidence assembled for
the purpose of evaluating coursework quality, learning progress, and academic achievement; determining
whether we have met the standards for courses we made. It helps us reflect and assess on the things that we
are doing. So in this module, it helps us to assess our work if we really are on track in doing the modules for
the LDM’s.
