Phân tích ca lâm sàng
1. Case 1
Cô L 45 tuổi được chẩn đoán mắc bệnh ung thư biểu mô buồng trứng giai đoạn
IIIc. Bác sĩ chỉ định cô dùng PACLitaxel và CARBOplatin, theo phác đồ
GOOVCATX.
Kết quả xét nghiệm trước khi điều trị:
• ANC 15.7 • Tiểu cầu 49 • Creatinine 79
Cân nặng: 73,4 kg; Chiều cao: 150 cm; GFR = 110 mL / phút (bằng thận đồ -by
renogram )
1. Những biện pháp phòng ngừa đặc biệt nào phải được tuân thủ khi chuẩn bị

Phân tích ca lâm sàng
1. Case 1
Bà G 54 tuổi dự kiến bắt đầu điều trị theo phác đồ điều trị LYCHOPR.
Bác sĩ ung thư của bà đưa ra kế hoạch điều trị là 6 liệu trình hóa trị theo phác đồ
LYCHOPR. Tiền sử của bà G điều trị bệnh ung thư vú 10 năm trước đó, sử dụng phác
đồ điều trị BRAJAC - hóa trị liệu đủ liều trong 4 liệu trình. Chiều cao và cân nặng
cũng như kết quả xét nghiệm của bà 1 tuần trước như sau:

PRECAUTIONS:
1. Neutropenia: Fever or other evidence of infection must be assessed
promptly and treated aggressively.
2. Myocardial ischemia and angina occurs rarely in patients receiving
fluorouracil or capecitabine. Development of cardiac symptoms including signs
suggestive of ischemia or of cardiac arrhythmia is an indication to discontinue
treatment. If there is development of cardiac symptoms patients should have urgent
cardiacassessment. Generally re-challenge with either fluorouracil or capecitabine is
not recommended as symptoms potentially have a high likelihood of recurrence which
can be severe or even fatal. Seeking opinion from cardiologists and oncologists with
expert knowledge about fluorouracil / capecitabine toxicity is strongly advised under
these circumstances. The toxicity should also be noted in the patient’s allergy profile.
3. Diarrhea: Patients should report mild diarrhea that persists over 24 hours or

moderate diarrhea (4 stools or more per day above normal, or a moderate
increase in ostomy output). If patient is taking capecitabine, it should be
stopped until given direction by the physician. Mild diarrhea can be treated
with loperamide. Note that diarrhea may result in increased INR and the risk
of bleeding in patients on warfarin.
4. Dihydropyrimidine dehydrogenase (DPD) deficiency may result in
severe and unexpected toxicity – stomatitis, diarrhea, neutropenia,
neurotoxicity – secondary to reduced drug metabolism. This deficiency is
thought to be present in about 3% of the population.
5. Possible drug interaction with capecitabine and warfarin has been
reported and may occur at any time. For patients on warfarin, weekly INR
during capecitabine therapy is recommended until a stable warfarin dose is
established. Thereafter, INR prior to each cycle. Consultation to
cardiology/internal medicine should be considered if difficulty in establishing a
stable warfarin dose is encountered. Upon discontinuation of capecitabine,
repeat INR weekly for one month.
6. Possible drug interaction with capecitabine and phenytoin and
fosphenytoin has been reported and may occur at any time. Close monitoring
is recommended. Capecitabine may increase the serum concentration of these
two agents.

chemotherapy.
NO PRIMARY SURGERY:
If no primary surgery was carried out, these patients are candidates for interval
debulking after three or four cycles.

nuclear renogram is available, this clearance would take precedence.

CNS Prophylaxis:
Patients with paranasal sinus involvement with large cell lymphoma who have a
complete response at the completion of their chemotherapy should receive
intrathecal methotrexate 12 mg alternating with intrathecal cytarabine 50 mg twice
weekly x 6 doses (3 doses of each over 3 weeks) starting in week 18. (See protocol
LYIT for details)
DOSE MODIFICATIONS:
1. Elderly Patients (age greater than 75 years):
Cycle 1 doses of cyclophosphamide and DOXOrubicin should be administered
at 75% doses. Further treatment should be given at the maximum dose tolerated
by the patient, trying to escalate up to full 100% doses, but using the baseline
experience with the 75% doses to guide these decisions.

1. Neutropenia: Fever or other evidence of infection must be assessed promptly
and treated aggressively.
2. Cardiac Toxicity: DOXOrubicin is cardiotoxic and must be used with
caution, if at all, in patients with severe hypertension or cardiac dysfunction.
Cardiac assessment is recommended if lifelong dose of 450 mg/m 2 to be
exceeded. (BC Cancer Drug Manual)
3. Extravasation: DOXOrubicin and vinCRIStine cause pain and tissue

necrosis if extravasated. Refer to BC Cancer Extravasation Guidelines.
4. Hypersensitivity: If applicable, monitor etoposide infusion for the first 15
minutes for signs of hypotension. Refer to BC Cancer Hypersensitivity
Guidelines. riTUXimab can cause allergic type reactions during the IV
infusion such as hypotension, wheezing, rash, flushing, alarm, pruritus,
sneezing, cough, fever or faintness. For first dose, patients are to be under
constant visual observation during all dose increases and for 30 minutes after
infusion is completed. For all subsequent doses, constant visual observation is
not required. Vital signs are not required unless symptomatic. Because
transient hypotension may occur during infusion, consider withholding
antihypertensive medications 12 hours prior to riTUXimab infusion. If an
allergic reaction occurs, stop the infusion and the physician in charge should
determine a safe time and rate to resume the infusion. A reasonable guideline
is as follows. After recovery of symptoms, restart riTUXimab infusion at one
infusion rate below the rate at which the reaction occurred and continue with
escalation of infusion rates on the appropriate schedule above. If the infusion
must be stopped a second time, restart after clearance of symptoms, at one
infusion rate lower and continue at that rate without further escalation. Fatal
cytokine release syndrome can occur (see below). See BC Cancer
Hypersensitivity Guidelines.
5. Fatal Cytokine Release Syndrome has been reported. It usually occurs
within 1-2 hours of initiating the first infusion. Initially, it is characterised by
severe dyspnea (often with bronchospasm and hypoxia) in addition to fever,
chills, rigors, urticaria and angioedema. Pulmonary interstitial infiltrates or
edema visible on chest x-ray may accompany acute respiratory failure. There
may be features of tumour lysis syndrome such as hyperuricemia,
hypocalcemia, acute renal failure and elevated LDH. For severe reactions,
stop the infusion immediately and evaluate for tumour lysis syndrome and

pulmonary infiltration. Aggressive symptomatic treatment is required. The
infusion can be resumed at no more than one-half the previous rate once all
symptoms have resolved, and laboratory values and chest x-ray findings have
normalized. The risk of cytokine release syndrome is low but is increased
when the peripheral blood lymphocyte count is greater than 30 to 50 x 10 9 /L.
While there is no requirement to withhold riTUXimab based on lymphocyte
count, clinicians may wish to pre-medicate patients with high tumour burden

6. Rare Severe Mucocutaneous Reactions: (similar to Stevens-
Johnson Syndrome) have been anecdotally reported. If such a reaction
occurs, riTUXimab should be discontinued.
7. Hepatitis B Reactivation: All lymphoma patients should be tested for both
HBsAg and HBcoreAb. If either test is positive, such patients should be
treated with lamiVUDine during chemotherapy and for six months
afterwards. Such patients should also be monitored with frequent liver
function tests and hepatitis B virus DNA at least every two months. If the
hepatitis B virus DNA level rises during this monitoring, management
should be reviewed with an appropriate specialist with experience managing
hepatitis and consideration given to halting chemotherapy.
8. Gastrointestinal Obstruction or Perforation: There have been rare
reports of gastrointestinal obstruction or perforation, sometimes fatal,
when riTUXimab is given in combination with other chemotherapy,
occurring 1 to 12 weeks after treatment. Symptoms possibly indicative
of such complications should be carefully investigated and
appropriately treated.
9. Medication Safety: riTUXimab is formulated differently for IV versus
subcutaneous administration. Use caution during prescribing, product

selection, preparation and administration. IV formulation is supplied as 10
mg/mL solution which must be diluted prior to administration. Subcutaneous
formulation is
supplied as a fixed dose of 1400 mg/11.7 mL ready-to-use solution which
contains hyaluronidase to facilitate injection.
10.Increased drug absorption by hyaluronidase: other subcutaneous
medications should not be injected at the same site as subcutaneous
riTUXimab. Increased systemic effects are unlikely to be clinically
significant with topical applications of EMLA, hydrocortisone, or
diphenhydrAMINE.

methotrexate levels and increased risk of methotrexate toxicity. Consider

administration. If their use if required, closely monitor methotrexate levels
and monitor for signs of methotrexate toxicity.
6. Possible interaction with penicillins (e.g., amoxicillin, piperacillin,
ticarcillin). Penicillins compete with methotrexate for excretion sites in the
renal tubules resulting in increased serum methotrexate and toxicity.
Primarily a concern with high-dose methotrexate and thus the combination
should be avoided if possible.

impairment, treatment should be discontinued.
PRECAUTIONS:
1. Skin toxicity: rash, erythema and acneiform rash are very common. In
general, rash manifests as a mild or moderate erythematous and acneiform
rash, which may occur or worsen in areas exposed to sun. Interrupt or
discontinue treatment if severe bullous, blistering or exfoliating conditions
develop as rare cases suggestive of Stevens-Johnson syndrome and toxic
epidermal necrolysis have been reported.
2. Diarrhea: diarrhea is very common and can result in acute renal insufficiency,
dehydration and severe electrolyte imbalance. Onset of diarrhea is within the
first 2 weeks of treatment, with grade 3 diarrhea occurring most frequently
within the first 6 weeks. Close monitoring and proactive treatment is essential.
Antidiarrheal agents should be readily available to patients so that treatment
can be initiated at first onset. Refer to the BC Cancer Drug Manual for detailed
management recommendations.
3. Ocular disorders: symptoms such as acute or worsening eye inflammation,
lacrimation, light sensitivity, blurred vision, eye pain and/or red eye should
be referred promptly to an ophthalmology specialist. For any diagnosis of
ulcerative keratitis, treatment should be interrupted or discontinued.
4. Interstitial lung disease (ILD): ILD or ILD-like events, including fatalities,
have been reported. Careful assessment of all patients with an acute onset
and/or unexplained worsening of pulmonary symptoms (dyspnea, cough,
fever) should be performed to exclude ILD.
5. Cardiac toxicity: decreased LVEF has been reported. If patients

develop cardiac signs/symptoms during treatment, a cardiac
consultation as well as treatment interruption/discontinuation should be
considered.
6. Gastrointestinal Perforation: Gastrointestinal perforation has been
reported, and approximately one-third of cases have been fatal. Patients with
history of gastrointestinal ulceration or underlying diverticular disease,
bowel metastases or those on concomitant medications such as anti-
angiogenic agents/steroids/NSAIDS are at increased risk; however, in some
cases, patients have had no known predisposing risk factors. Permanently
discontinue AFAtinib following development of gastrointestinal perforation
7. Drug interactions: AFAtinib is a substrate of P-glycoprotein (P-gp). Strong
inhibitors of P- gp, if administered prior to AFAtinib, may lead to increased
exposure to AFAtinib and should be used with caution.

PRECAUTIONS:
1. Pamidronate should NEVER be given as a bolus since severe local
reactions and thrombophlebitis may result from high concentrations.
2. Symptomatic hypocalcemia (e.g., muscle spasms, irritability) may occur
and may require calcium supplement. Avoid concomitant use of other
calcium lowering agents such as corticosteroids and loop diuretics.
3. After the use of bisphosphonates, there is a persistent risk of jaw
osteonecrosis. Patients in whom bisphosphonates are planned should
have prophylactic assessment and management by a dentist and all
later dental work should be undertaken cautiously by dental specialists
experienced in the recognition and management of jaw osteonecrosis
4. Duration of treatment: The BC Cancer Breast Systemic Tumour Group
recommends a maximum continuous exposure of patients to
bisphosphonates of 2-3 years, due to increasing incidence of atypical
femoral fractures with prolonged use. However patients may be treated
for longer if additional clinical benefit is likely in the judgement of their
treating oncologist.

LESSON 11
ELIGIBILITY:
Postmenopausal women and men with ER-positive, HER2-negative
advanced breast cancer with no prior systemic treatment (including
chemotherapy) for metastatic disease (including women with chemically
induced menopause with LHRH agonists).
Patients should not be resistant to prior (neo)adjuvant aromatase inhibitor
therapy (patients must be a minimum of 12 months from last adjuvant
aromatase inhibitor), nor have active or uncontrolled metastases to the
central nervous system.
