III. PROBLEM PRESENTED:

Mrs. Charlita personally came to this office requesting a Social Case Study Report for her
aunt’s medical assistance from the government and non-government agencies.

IV. FINDINGS:

Client disclosed that prior to subject’s confinement, she felt abdominal pain. And so due to
severe pain she felt, she was then rushed to __________________ Hospital. However, because of her
condition the doctor refer subject to ______________ Hospital. She was then admitted at the said
hospital for two weeks. The physician diagnosed patient with ACUTE KIDNEY DISEASE and advised
patient and family that she needs to undergo for operation. But then due to unavailability of the
procedure patient was then brought to Medline Dialysis Center. Attending physician at Medline
Dialysis Center diagnosed patient to have a CHRONIC KIDNEY DISEASE STAGE V SECONDARY TO
HYPERTENSIVE KIDNEY DISEASES. The physician there advised patient to undergo maintenance
dialysis at out institution 3x a week for the betterment of her condition. Accordingly, subject has
already undergone number of dialysis.
 As per interview, client averred subject was married with her late husband and they have 8
children. Client added that subject raised her children alone when her husband died while her
youngest was 5 years old then. Accordingly, among her 8 children 4 among them has a family of their
own. Subject’s family is presently living at Barangay_________________. Client revealed that only
Mark (6th son of the subject) earns money in the family. Client further revealed that subject’s family
has no investment and even properties that could help for her maintenance dialysis. She has to take
dialysis 3 x a week, and they could hardly provide due to financial difficulty.

V. EVALUATION AND RECOMMENDATION:

This is a case of Mrs. Trini V. Casio who is undergoing maintenance dialysis 3x a week due to
his illness and family has no enough earning to pay the said medication. Client disclosed that
subject and his family has no capacity to shoulder the treatment procedure since only the 6th son of
the subject is the breadwinner of the family and there is no other resources.

As per interview and information gathered from Mrs. Charlita, subject Mrs. Trini V. Casio, was
found in need of assistance. The worker would like to recommend client/subject for possible
help/assistance, which your institution/agency could best extend.
