SUBJECT : After Activity Report re: Disinfection and Sanitation of

PDL Dormitories

DATE : 17 September 2021

I. Authority

1. Reference: Adaptation of “tuob” as per verbal instruction of JCSUPT

ARNOLD F BUENACOSA, Regional Director of the Jail Bureau Region VI to
the Jail Warden to keep our facility free from Covid-19 infections.

2. In connection with the above reference, OOA 0700H of 17 September

2021, NODJMD Locked- in Personnel conducted disinfection and sanitation of
dormitories using of Fog Machine to eliminate bacteria and viruses.

3. Below are the chronological order of events that transpired during the
said activity.

September 17, 2021 Locked in duty personnel of this unit conducted
0700 disinfection and sanitation of Dorm 3 and 6 .

0705H Start of the above mentioned activity

0800H End of activity.

reference.

III. Observations and comments

2. While the conduct of the said activity was successful, hereunder are the
Observation and comment, as follows:

a. Activities ended with no untoward incident happened.

b. Such activity help maintain a healthy environment.

IV. Recommendations

3. Hereunder is the recommendation to consider, as follows:

a. Continuous conduct of such activity to disinfect and sanitize

dormitories to eliminate viruses and bacteria.
