Anggun (Ns) : Good evening miss, how are you feeling today?
Cintya (Px) : Good evening nurse, I am okay
Anggun (Ns) : You must be feeling really happy to be going home today
Cintya (Px) : Yes, but I am a little worried about my condition
Anggun (Ns) : Well, don’t worry I give you recipe from the doctor
Cintya (Px) : Okay Nurse
Anggun (Ns) : I have to do discharge summary today miss
Cintya (Px) : Yes nurse
Anggun (Ns) : You need to come back after 7 days to check your condition
Cintya (Px) : What time nurse?
Anggun (Ns) : Anytime, in the morning is fine it's very quickly process okay
Cintya (Px) : Yes nurse
Anggun (Ns) : Okey, let me just quickly show you the medicines that you are
supposed to be taking okay, this is the entire list of medicines. Are
you understand miss?
Cintya (Px) : I understand nurse
Anggun (Ns) : Okay. I will give some information to your family about diet rule
which are to avoid salty dishes, offal, coconut milk and doing
regulary exercise. Sign of hyperglycemia such as feeling thirsty
and excessive hunger, frequent bowel movements, weakness,
fatigue, lethargy, blurred vision and headaches.
Elvi (Fam) : Okay thank you nurse about your information.
Anggun (Ns) : Okay Mrs. Elvi. Maybe there is something you want to ask again?
Elvi (Fam) : No thankyou
Anggun (Ns) : Okay so if any something wrong please come back to the hospital

(Nurse in the hospital call nurse community to do continuing care for patient)

Endah (Ns) : Hello good morning. I am Ns. Endah from Brawijaya hospital. It's
puskesmas sumbersari right?
Lisa (Ns Com.): Hello good morning. Yes that's right.
Endah (Ns) : I would like to tell you that our patient Miss Cintya had left from
our hospital. Can you send your nurse to give continuing care for
this patient?
Lisa (Ns Com.): Of course we can. Please give the patient detail information.
Endah (Ns) : It's Miss Cintya she got diabetes melitus. She lives at Jl.
Kalimantan No. 7
Lisa (Ns Com.): Okay thank you

(Nurse Community called the patient to ask when she can meet the patient to
do continuing care)

Lisa (Ns Com.): Good Morning, i am Community Nurse from Puskesmas

Sumbersari. Brawijaya Hospital asked me to be your Nurse that
looking after you during home caring. Is it right that im talking
with Miss Cintya?
Cintya (Px) : Good morning, Nurse. Yes, Iam Miss Cintya. Oh yes, Nurse
Anggun have told me about it before
Lisa (Ns Com.): Well Miss Cintya how about your condition today?
Is there any problem with your body?
Cintya (Px) : I am feeling well Nurse. I think my condition get better.
Lisa (Ns Com.): Do you have any spare time ? So i will go to your home
Cintya (Px) : Sure, anytime you can come to my home
Lisa (Ns Com.): Okay, i will make the schedule. Well Miss Cintya, i will go to
your house on Tuesday at 9 a.m.

(The Nurse goes into the house and meet the patient)

Lisa (Ns Com.): Excuse me Mrs, Good morning. Iam Nurse Lisa as a community
nurse from Puskesmas Sumbersari that will checking your blood
sugar and controlling your diet
Elvi (Fam) : Good Morning too Nurse. Have a seat please.
Lisa (Ns Com.): Okay thankyou. Can I meet Miss Cintya?
Elvi (Fam.) : Okay, wait for a minute. I will call her.
Lisa (Ns Com.): Okay
Cintya (Px) : Excuse me, Iam Cintya. Is it Nurse Lisa that called me to check
my condition?
Lisa (Ns Com.): Yes, it is me. How is your condition this morning ?
Cintya (Px) : I am in a good condition, Nurse.
Lisa (Ns Com.): Then, i will checking your blood sugar, first.
Cintya (Px) : Sure, Nurse.

(The Nurse checking blood sugar on patient)

Lisa (Ns Com.): After I did a blood sugar test to you, the result is normal.
Cintya (Px) : Alhamdulillah, Iam glad to hear that.
Lisa (Ns Com.): And how about your diet since you have this diabetes?
Cintya (Px) : I obey the diet regulation from the nurse that told me before
Lisa (Ns Com.): Good job miss, and I hope you consistent to do it. If you keep
your diet, you will get better and your blood sugar wil be stable.
Cintya (Px) : Sure Nurse, I will keep my diet well.
Lisa (Ns Com.): Ok miss, I think enough for today and I will coming again on
Friday Miss.
Cintya (Px) : Ok, thank you Nurse.
