CREATING THE LGU GRIEVANCE AND APPEALS COMMITTEE OF THE MUNICIPALITY OF

_________________ FOR THE DISTRIBUTION OF FINANCIAL ASSISTANCE TO SEVERELY
AFFECTED BY TYPHOON ODETTE

WHEREAS, On December 16, 2021, Typhoon Odette caused widespread devastation and
damage to houses, infrastructure, properties and agriculture, which resulted in the disruption of
millions of lives within our city/municipality.

WHEREAS, On December 21, 2021 the Office of the President issued Proclamation No.
1267, Series of 2021, “Declaring A State of Calamity in Regions IV-B, VI, VII, VIII, X and XIII. Such a
declaration will hasten the rescue, relief and rehabilitation efforts of the government and the
private sector, including any international assistance.

WHEREAS, On December 22, 2021, Cabinet Secretary Karlo B. Nograles during the press
briefing in Davao City announced that the President ordered the release of funds to assist all
those who were severely affected by Typhoon Odette.

WHEREAS, on December 28, 2021, DBM issued Local Budget Circular (LBC) No. 141
which will serve as the implementing guidelines and budget parameter for the said financial
assistance.

WHEREAS, on December 29, 2022, the Department of the Interior and Local
Government, the Department of Social Welfare and Development, and the Department of
National Defense issued a Joint Memorandum Circular (JMC) no. 4, series of 2022, for the
distribution of financial assistance to cities and municipalities in the amount of One Thousand
Pesos (Php 1,000.00) per affected individual, with a maximum amount of Five Thousand Pesos
(Php 5,000.00).

WHEREAS, to address the issue of complaints, grievances and appeals of the

constituents in the distribution of financial assistance, DILG-DSWD-DND JMC no. 4 orders to
create a Grievance and Appeals Committee.

NOW THEREFORE, I ___________________________ City/Municipal Mayor of

______________, by virtue of the power in me by law, do hereby order as follows:

Section 1. Creation of Grievance and Appeals Committee. The Grievance and Appeals
Committee (GAC) shall be created and composed of the following:

Section 2. Functions. The Committee shall perform the following functions:

a. Address the complaints, grievances and appeals coming from the constituents in
relation to the distribution of financial assistance to severely affected families brought
by Typhoon Odette;
b. Create similar committees at the barangay-level, if necessary;
c. Shall act expeditiously on all complaints;
d. Maintain a hotline to receive complaints;
e. Create a complaint desk to cater all walk in complainants; and
f. Perform other task as may be needed

Section 3. Effectivity.

This order shall take effect immediately.
