DAILY ACTIVITY REPORT

Day 1 (July 21, 2022)

First day of On Job Training. Before we go to the assigned site, Solid M Construction
distributed our PPE’s and tours us to the office of the firm. It was 10am in the morning when we
arrived at the site. Before we proceed for inspection/site seeing, we met the Municipal Safety
Officer and he discusses what are the safety rules and precautions in a construction site to
prevent accidents and unnecessary events. After discussing, they gave a little detail what is
currently happening/constructing in the site and showed us the full plan of the project.

In the afternoon, Ma’am Grace discusses what are the allowed and prohibited activities
inside a construction site. She also discuss the most important is that we must always wear our
PPE when observing to avoid any accidents. The project is just starting, and I’m expecting to
learn more as the project progress.

Day 2 (July 22, 2022)

2nd Day of OJT. Site Engineer Ma’am Grace Gonzaga introduces us to the project, who
will occupy it, how will they construct the building and the materials used in the construction.
Ma’am Grace Gonzaga introduces to us who is the foreman and some skilled workers in the site.
The foreman also shared some details about the structure and some information that can help us
understand what they are doing. After lunch, she tours us around the site and showed us the
equipment they are using, for example is the H-Frame Scaffold. H-frame scaffold is used for slab
and others. One set of H-Frame Scaffolding system consists of one H-Frame, one horizontal
Bracing, and two Diagonal Bracings.

Day 3 (July 21, 2022)

It was 8am when I arrived at the site. After wearing the PPE, I immediately checked the
Lean Concrete that was made yesterday. Main function of the lean concrete is to provide the
uniform surface to the foundation concrete and to prevent the direct contact of foundation
concrete from the soil. As I expected, the lean concrete is just partially (dry) because of the
overflowed water in the surface of the concrete. After that, I went to observe the construction of
the beam. During break time (9am) Ma'am Grace discuss about our assignment, about the other
safety rules and precautions in a construction site. There are a lot of rules but we discussed the
main or the rules that can be apply here in the site, example of this is the proper wearing of
safety gears. (Helmet, gloves, Vest), proper placement of tools and materials and etc.

After a while, they are now preparing for the placement of the bed bars of the footing. I
observe that there are little concrete attach to the bed bars and this is Called Spacer Block.
Spacer Block is little concrete parts which are used to position the rebar. I am also observing on
how they build the formworks for foot tie beam and footing of the structure so it will be ready
for cement pouring.

Day 4 (July 25, 2022)

Monday, July 25, 2022. It was 6:58 when I arrived at the site. Every Monday, they are
conducting a meeting called toolbox meeting. This meeting talks about what happened for
the past days and other important things in the site. After the meeting, we immediately
proceed to the structures. The foreman thought us the details about the B1 , B2, B3 , lintel
Beam, Foot tie beam, Footing , splicing, H-Frames, cut bars, web bars, bottom bars, stirrups
and many more. The foreman also thought us of how is he managing such a big project and a
large man power, being a foreman is an important role because you need to manage a large
number of workers and lead them. Today, they are making the formworks of the foot tie
beam and the footing so that they can pour concrete today or tomorrow. After a couple of
hours, we move to the making of formworks for slab using the phenolic board. After
attaching the phenolic board, they are applying marine epoxy on the side of the board.

In the afternoon, i observe how are they cutting the bars and forming the splicing in
beams. It was a rainy afternoon, and they are targeting to finish the formworks of other foot
tie beam so that they can pour concrete later in overtime. They are also targeting to finish the
tie beam so they can do concrete pouring for the slab tomorrow. 2pm, foreman talks about
how are they preparing for slab structure, scaffolding, and other materials for 2nd floor.

Day 5 (July 26, 2022)

July 26, 2022, when I arrived at the site, foreman, municipal safety officer and Municipal
Engineer showed us on what is the right way of wearing a safety harness. A safety harness is a
form of personal protective equipment (PPE) that is designed to catch a person in case of falling
while working at height. The 3 workers are now required to wear harness because they are 6ft
above the ground splicing the mid column. They also discussed what the importance of harness
is and how it saves so many lives in a construction site.

In the afternoon, they started backfilling. Backfilling is the process of putting the soil
back into a trench or foundation once excavation, and the related work has been completed.
During that time, we asked what are they using in compacting the soil, the foreman showed us
their Plate Compactor and Jumping Jack.

Day 6 (July 27, 2022)

July 27, 2022. They continue in backfilling and started working for formworks of
columns, footing and footing tie beam. They also are preparing the scaffolds for the slab in 2nd
floor and tie beam. The other workers are applying used oil in phenolic boards to be used for
formworks. Later on, we just continue to observe and take notes about cutting of bars and
dimensions of stirrups and how many stirrups they are using in every tie beam. They continue in
constructing the foot tie beams because they are waiting for the supply of 20mm steel bars.

After lunch, we observe the process of splicing in column and beams. Foremen also tell
us how he is going to do the concrete pouring for the slab in the 2 nd floor of the building. He also
share some experience in construction that can help us understand more about construction.

Day 7(July 28, 2022)

Early in the morning, they started compacting the area using Jumping Jack Compactor.
During compacting, the foreman and the Municipal Inspector is checking if the soil is fully
compacted. The other workers continue in splicing the in columns. Today, we just continue in
computing our estimates of the project.

In the afternoon, there was a problem of the jumping jack compactor. It keeps on shutting
down and hard to start up. The work for compacting is delayed for a couple of hours. They are
also preparing the wooden formworks for beams because they are targeting to make the slab on
Saturday.

It was a rainy day. Ma’am Grace discussed about the incident yesterday at the site. She
also checked our estimates and daily activities. I went to the site and they continue compacting
the area, constructing the formworks for beams, columns and slabs. Also, they are finishing the
beams so that it will be ready for Saturday concrete pouring. Before lunch break, Ma’am Grace
checked the site if there is any concern or problem during the day.

In the afternoon, they attach the wooden formworks in beams for the 2nd floor slab.
Before Ma’am Grace went to office, she gives us a task to make a list of equipment used in
construction site or equipment we use in the site. Furthermore, foreman thought us the exact
dimensions of lateral ties and stirrups used and show us how to compute the dimension.

It was 9am when Ma’am Grace arrived at the site. She reminded us about the daily
activity and our assignment which is the estimates. During that morning, Engr. Martin Junboy
Tan was at the site and tells us to always wear our PPE's every time we are going the site for our
safety. They continue on cutting bars, making formworks and attaching it to the beams and for
slabs. They are now preparing for the concrete pouring of slabs in the 1st week of august. After
that, we continue to compute on our estimates.

In the afternoon, we saw the assigned plumber that he started to work on sanitary
installation. As the day continues, the materials (steel bars) arrived at 2pm in the afternoon. As
we wait, we are watching the procedure of cutting and bending of bars. They are targeting to
finish the beams, columns, and footing tie beam on the 1 st week of august so they can proceed on
slab for the second floor of the building.

Monday, August 1, 2022. It was 7:01 when I arrived at the site. Every Monday, they are
conducting a meeting called toolbox meeting. Today’s meeting, the foreman discuss about the
workers misplacing their PPE during work. Ma’am Grace discuss about their new target this
week. That they will have OT from August 1 until they finish the slab in second floor because
Engr. Martin Junboy Tan (The Owner) give them a deadline of the project. Later in the morning,
the rain started to pour but still they continue to work on formworks, cutting/bending of bars and
splicing of bars.

Late in the afternoon, the rain keeps on pouring. They add another span of beam for the
slab. When the rain stops, we observe their work in cutting/bending of bars and take notes of the
dimensions and sizes of bars. During overtime, Ma’am Grace thought us some heads up on how
to do an oral presentation of our daily activities on the site.

2nd day of August. Early in the morning, they started to add another span of beam. During
that morning, I observe them on how they are attaching the wooden formworks to beams. I also
observe the cutting and bending process of bars. Before lunch, Ma’am Grace told us to finalize
our computations on estimates. Before lunch, the other materials arrived. Foreman taught us on
how to use the angle bars that recently received.

In the afternoon, they are finishing the added span of beam and continuing making of
formworks, scaffolds, stirrups and cutting/bending of bars. While watching, Foreman and the
steel man taught us on how to use the bending machine and cutting machine. Later in the
overtime, they just continue on splicing and constructing the added span of beam of the structure.

Today, we just continue how they are constructing the beams, formworks,
cutting/bending of bars and splicing. During that time, we take notes on how they are planning to
build the slab for next week. Before Ma’am Grace leave, she reminded us that tomorrow (Aug. 4,
2022) will be our deadline of our daily time sheet, computations for estimates and other
requirements for Solid M company. The other workers continue to attach rebar on beams.
Today, we just continue in computing our estimates of the project.

This afternoon, they started to arrange the phenolic boards (formworks) in the 2nd floor
for the slab and attach other formworks for beams. Foreman give us permission to climb and see
what is the process of forming the formworks of slabs and how are they going to do the concrete
pouring for next week.

Day 13(Aug 4, 2022)

This is our last day before shifting. Early in the morning, I immediately check the site for
progress. I observed that they added another span of formworks for slab and started
cutting/bending of bars for bed bars. They also continue constructing the remaining beams for
6span slab. Late in the morning, it started to rain because there is a LP (Low pressure) but it
didn’t affect the workers in the site.

During afternoon around 2pm, it started to rain heavily. Some of the workers that
constructing the formworks and beam need to stop because of the heavy rain. As the time goes
by, they stop their workload and wait for the rain to stop. Before 5pm, foreman told us that they
can’t go overtime because of the weather condition. We just continue to compute in our
estimates and went home after 5pm.

First day of shifting, we went to the Solid M office and wait for the schedule to be
release. That morning, Ma’am Pearlyn announced where is our next assignment will be and wait
for our Site Engineer to arrive. 10am, we went to the site which is located at University Of
Eastern Philippines. In the Afternoon, Engineer Jonathan Bulagao gives us an assignment to
make a cutting list of the project and compute for the estimates.

Saturday morning, the time keeper informed us that Engr. Jonathan Bulagao will not
come to the site. So he instructed us to continue computing on our cutting list and estimates and
have a site seeing for the rest of the day. During our inspection, I saw some new materials that
were not in the previous site I was assigned, so I took some pictures and take some notes during
that day.

Early in the morning, Engr. Bulagao tours us to the site and gives some information about
the project and the materials they used. There are some terms that is new to me, like the aluma
beam, pin connector, cross bracing, swivel clamp, shoring jack and different kinds of department
need in a construction. He also discusses the importance of every department. In the afternoon,
we just continue to compute in our estimates and try to finish it before 5pm.

We visit the site again early in the morning and have some discussion about our questions
about our estimates and cutting list. Before Engr. Bulagao went to office, he left us another
assignment to practice our computations in structures. During afternoon, I observed some
workers working on a landscape and take some notes and pictures during the process.

We visit the site again early in the morning and have some discussion about our questions
about our estimates and cutting list. During that day, they continue attaching the steel deck for
the slab and Engr. Bulagao discusses the uses of steel deck. After a few minutes, we went to the
other site and observe how they are pouring concrete for the foundation of the structure and the
footing columns. Engr. Bulagao let us stay for a while to observe. In the afternoon, we just
continue to compute for our estimates and cutting list.

In the morning, we went to the site and Engr. Bulagao discuss about the common
mistakes or error in a structural design. As we do every day, we went to the second site and
observed. Today, they are making the footing tie beam of the structure and Engr. Bulagao asks
some questions about our cutting list. In the afternoon, we continue to compute for the cutting
list and estimates. Before 5pm, we ask permission to Engr. Bulagao if we can have our overtime
in Shoppers Ville (our previous assigned site) because today is their concrete pouring for the slab
in the second floor and we want to observe the process.

When we are at the site, Engr. Bulagao ask some questions about what do we observe in
the structure and what will you do if you encounter some problems in the structure, he also
discuss some safety protocols inside a site construction. When we visit to the second site, the
workers just continue building the footing tie beam and the others are cutting bars. In the
afternoon, Engr. Bulagao left to take care of the materials that will be using in the second site.
We just stayed in our room and finishing our computation on estimates and cutting list.

This Saturday, Engr. Bulagao went to tacloban for some important matter. In the
morning, we stayed in our room and trying to finish our estimates and cutting list. In the
afternoon, the 3 of us and the foreman went to the second site and ask some questions that could
help us in our assignment. He gives some information and insights of the structure. After that, we
went back to our room and continue working on our assignment.

Early in the morning, foreman said that he will visit the second site, so we stay in the
office because the timekeeper is not around. We need to keep the materials checked if someone
wants to take or borrow. When Engr. Bulagao arrived, we check the site for a short time and
went back to the office so we can finish our estimates and cutting list. In the afternoon, we just
continue working on our assignment and take some notes that we learn that day.

When I arrived at the office, foreman and the others workers is discussing about the slab
in the plan and we tried to help and understand. We went to the rooftop and check the site with
Engr. Bulagao and ask some questions about the project. After an hour, we went back to office
and Engr. Bulagao gives us another assignment which is the estimation of scaffolding, aluma
beam and etc. In the afternoon, we went to the site and start observing and computing for the
estimation of scaffolding and etc.

When Engr. Bulagao arrives, we immediately went to the site and check on the roof for
the slab. When we asked the foreman, they are targeting to concrete pour for the slab on friday.
During that day, foreman focuses on the work for the slab because of the deadline given to them.
After an hour, Eng. Bulagao went to the office to take care of the materials and we interns
continue to work on our assignment. In the afternoon, foreman visits the other site and told us to
take care of the materials inside the office.

Today, we just stay in the office because the foreman is going to visit the other site and
we need to take care of the materials inside the office. Engr. Bulagao arrives and checks on the
work on the rooftop. We also monitor the area because some outsiders/students are taking
pictures inside the building. Students are still not allowed because it is still under construction
and also dangerous for any accidents may happen.

Today, the interns assigned in Shoppers Ville will join us to inspect the site. Engr.
Bulagao tours us around the building and let us asks some questions about the project. After that,
Engr. Bulagao gives us a task to find the right key on every room in the building including the
electrical and comfort room. Take us a couple of hours but we didn’t find all the keys of every
room in the building. In the afternoon, the interns from Shoppers’ Ville help us on our
assignment and give some important details about the structure.

Today is my last day of OJT and I’m still trying to learn new things. Today is a very busy
day because they are targeting for the concrete pouring on Monday next week. We inspect the
scaffolding they use so we can finish our estimates. We also observe how they setup the metal
pipes and the pumpcrete truck. The foreman and Engr. Bulagao was very busy so we stay at the
office to take care of the materials inside the office.
