Parents’ Inquiries/Concerns:
This is the first week of the Third Quarter’s distribution and

retrieval of modules.
Some inquiries of the parents are the following:
 Grades of the students for the previous quarter

 Requirements for Early Enrollment
Challenges Encountered:
 3 students were not able to receive modules for the past

two weeks
 Some students don’t have grades in other subject areas
 Late submission of modules
Solutions/Suggestions/Recommendations given:
 Released the cards for parents to check the grades

 Provided early enrollment requirements
 Asked parents to talk to the subject teachers to check
with their children’s performance
 Send letters to parents
Achievements:
The adviser was able to release ___ modules out of 38 students. Some
parents were absent during the distribution, but the adviser made
sure to leave the modules on the designated pick up points with
complete instructions.
The adviser also received ___ modules and answer sheets from parents
on Friday of the said week.

importance of submitting the modules on time

with their children’s performances

Quarter: III
Parents inquired about the scores of their children in the

given activities.
Some parents also show their concern on the result of their

children’s summative test as they learned that those are the
basis of the students’ grades.
Others also asked on how to cope with absences and failed

grades in a specific subject area.
The adviser finds it challenging to distribute modules to

students whose parents are absent during the assigned
day. She already provided pick up points where they can
get the modules, but there are still students who are not
able to pick up their modules.
The send letters to the parents who were absent during the
distribution of modules and those who failed in picking the
modules from the designated pick up points.
Achievements:
The adviser was able to release 30 modules out of 38 students. Some

Quarter: III
Parents’ concerns include the number of modules they received

as they are few compared to the previous quarter. They also
inquired about the possibility of their children to be in
school to received and return the modules instead of them.
The weather is fine the entire week, thus parents are early
during the distribution and retrieval of modules but there are
still parents who failed to show up.
The adviser sent letters to the parents who were absent

during the distribution of modules and those who failed in
picking the modules from the designated pick up points.

Quarter: III
Some of the parents inquire about the scores of their children

as it’s almost the end of the 3rd Quarter. They are also
concern about their children’s low performance and asked about
the summative tests provided by the subject teachers.
It rained throughout the day thus there are only few numbers
of parents who came to retrieve and return the modules of
their children, which is concerning as there are several
modules that arrived the previous day.
The adviser advised the parents to talk to the subject

teachers with regards to their children’s performance. She also
provided them with the students’ grades, especially those who
performed poorly during the Quarter this is to make sure that
the parents are aware and will help their children be on track.

Quarter: III
Parents of the students who performed poorly during the

Quarter are called and provided with the information with
regards to their children’s performance. They inquired about
the remediation and how it will be done.
Parents who were not able to come the previous week are
present to retrieve and return the modules. Yet the adviser
still finds it challenging to send modules to those parents
who are absent during the period.
The adviser coordinated with the subject teachers as to

how the remediation for those students who performed poorly in
the quarter.
She advised the parents to assist the students in

answering the activities given to them by the subject teachers.
