A Semi Detailed Lesson Plan

In English V

I. Objectives
By the end of the lesson, the pupils are expected to:
a) fill out forms accurately and
b.) appreciate the importance of filling out forms accurately

II. Subject Matter

A. Topic: Filling out forms Accurately (School forms, deposits slips and withdrawal slips)

C. Materials:
 Forms, Laptop, PowerPoint presentation, and video presentation.

III. Procedures

A. Preparation
The teacher will begin the class by greeting the students, leading the prayer, and
checking the attendance.

B. Motivation
The teacher will give a form to be fill out by the pupils.

C. Lesson Proper
  The teacher will start by presenting the lesson through PowerPoint presentation
about the parts of the withdrawal and deposits slip and some examples.
 The teacher also resent the lesson by letting the pupils watch a video on how to
fill up forms.
 The teacher will be using a Venn diagram to show the similarities and differences
of the deposit slip and withdrawal form. This will help the pupils to fully identify
and learn the differences and similarities of the two forms.

D. Activity
The students will identify the parts of the atm to be able to fill out the withdraws
slip.

E. Generalization
To fill out a form is to complete a form by writing the required information
on it. There are particular forms that you have to fill out as a
requirement for admission to a school or when transacting in a bank. Accuracy is
very important in filling out these forms.

F. Evaluation
The teacher will an activity sheets to the pupils.

IV. Assignment
The students will fill out their withdrawal slips given by the teacher.
