Extrajudicial Settlementof Estate
WITHNESSETH:
That Barbara V. Young, Loretta V. Mendoza and Norma V. Mendoza are the
daughters of the deceased Alfredo Mendoza. All of them are the legitimate and sole heirs of the
deceased Alfredo Mendoza who died on __________________ at _____________________
without a will;
That the deceased Alfredo Mendoza, at the time of his death, left real properties
more particularly described as follows:

That to the best of information and knowledge of the parties hereto, the deceased
Alfredo Mendoza left no debts;
That this Extrajudicial Settlement of Estate of the deceased Alfredo Mendoza had
been published one week for three consecutive weeks in the Bicol Post, a newspaper of general
circulation in the Province of Camarines Norte as certified by said newspaper hereto attached
as Annex “A" of this instrument.
That the parties hereto, being all of legal age, with full civil capacity to inter into a
contract, hereby extra judicially settle the estate of Alfredo Mendoza pro –indiviso and
adjudicate unto themselves all the above-described real properties of the estate pursuant to
Section 1, Rule 74 of the Rules of Court.

BEFORE ME, a notary public for and in the Province of Camarines Norte personally
appeared the above-named parties and presented their respective valid IDs/Community Tax
Certificate, thus:
Known to me to be the same persons who executed the foregoing Extrajudicial Settlement of
Estate pro-indiviso and acknowledged the same to be their own voluntary acts and deeds.
I further certify that the foregoing instrument consists of _____ () pages including this
page on which this acknowledgement is made, signed by the parties and their instrumental
witnesses at the bottom of the instrument on page ____ and on the left hand margin of the
remaining pages.
