Section A: Technical Proposal

1. INTRODUCTION
Consultation, Research & Development Department (CRDD), NIOSH was invited by SYARIKAT ONG YOKE LIN SDN. BHD. to propose
Confined Space Risk Assessment in its working areas.

2. BACKGROUND OF NIOSH
National Insitute of Occupational Safety and Health, Malaysia (NIOSH) was established in 1992 by the government
(Ministry of Human Resources) to spearhead safety and healthy culture at the workplace in Malaysia. Located in
Bandar Baru Bangi as registered office and headquarters for operations with 4 other regional offices, namely Northern
Region Office, Southern Region Office, Eastcoast Region Office and East Malaysia Region Office.
NIOSH core activities are as follows:
i. Develop and conduct training programmes
ii. Assist industries, commercials, and others in solving problems related to OSH through consultancy services
iii. Provide latest information in the field of OSH
iv. Conduct applied research and development activities
v. Examination and assessment for competency courses (e.g. Safety and Health Officer, Industrial Hygiene T echnician, Chemical Health
Risk Asssessor, Occupational Health Doctor, Occupational Health Nurse, Authorised Gas T ester for Confined Space, Forklift Drivers,
Occupational First Aiders and etc.)

Consolidation of highly technical OSH consultation such as Regulatory Compliance (Industrial Hygiene & Occupational Health
services), OSH solutions (Occupational Safety & Management) and Advisory services (ergonomics and occupational safety).

3. GOALS
The objectives of this monitoring are:
NIOSH was invited by SYARIKAT ONG YOKE LIN SDN. BHD. to pr opose consultation ser vices on Confined Space
Risk Assessment Consultation in its working areas. The objectives of this project are:
To evaluate the current occupational safety and health (OSH) legal compliance of the work activities for
confined space;
To assess the risk of work activities for confined space; and
To recommend preventive measure to improve safety and health of workers.

4. SCOPE OF WORK
NIOSH shall conduct this assignment by the step include the followings:

5. RESPONSIBILITY
SYARIKAT ONG YOKE LIN SDN. BHD. is responsible to provide relevant documents as per Self Assessment ICOP Confined
Space Checklist for Workplace 2021 in Appendix A
6. GOALS
DELIVERABLES
Upon completion of the project, NIOSH will:
Deliver an official report of CSRA within one month after the last day of agreed draft report.

7. PROPOSED PERSONNEL
NIOSH shall include the provision of all the necessary approved, suitably qualified and competent personnel to carry
out and complete the project.

C. TERMS AND CONDITIONS

It is expected that any contract arising from this Proposal will be between company and NIOSH will be conducted
under the Terms and Conditions agreeable between both parties. Specific terms and conditions as follows:
Validity is valid for sixty (60) days from the date of Proposal.
NIOSH will submit the invoice to the client after completion of final ']
If the project value more than RM20,000.00 client is required to make 20% upfront payment to NIOSH
If the project value less than RM300.00, client is required to make payment in cash (COD)
Terms of Payment within 14 days from the date of tax invoice.
All fees are subject to 6% SST.

internet-based portal designated by NIOSH or its affiliates, and/or other
electronic means), including, without limitation, those that would
otherwise be sent via Malaysia or international mail, messenger,
courier, or similar service. Examples of “electronic documents” that
may be exchanged in this manner shall include, without limitation,
client agreements and amendments thereto, general correspondence,
notices, instructions, account information and statements, billing
invoices, receipts, disclosures, reports, consents, consultation
recommendations or advice, and brochures. Client may revoke this
consent to electronic exchange of documents at any time, on written
notice to NIOSH.
2. Consent to Use of Electronic Signatures.
By completing this form, SYARIKAT ONG YOKE LIN SDN. BHD. is
providing electronic consent to the use of electronic
signatures.
An "electronic signature" includes any mark, symbol, sound, or
process that is written, stamped, engraved, attached to or logically
associated with an electronic document executed by a person with the
intent to sign. This includes paper documents signed in ink, saved, and
transmitted electronically by SYARIKAT ONG YOKE LIN SDN. BHD.. Client
agrees that an electronic signature shall have the same force and effect,
validity, and legal enforceability as a manually executed signature to the
fullest extent permitted by applicable law. SYARIKAT ONG YOKE LIN SDN.
BHD. may revoke this consent to use of electronic signatures at any time,
on written notice to NIOSH. The validity of any electronic signatures
entered prior to NIOSH receipt of notice of revocation of consent to use of
electronic signatures shall be unaffected by such revocation.
3. Access to Electronic Documents and Signatures.
SYARIKAT ONG YOKE LIN SDN. BHD. acknowledges that to access, retain,
electronically sign (where requested) and print paper copies of electronic
documents, SYARIKAT ONG YOKE LIN SDN. BHD. must have access to
an e-mail address, computer/tablet/smart phone or other similar
internet connected device (with web-browsing software and
sufficient digital storage space), a printer, and a functioning internet
connection. By signing below, the client acknowledges having access to all
