1. On August 28, 2021 at around 2:00 pm, a concerned citizen Mario

Luigi (not his real name) appeared to the office and reported a fire
incident in Barangay Little Tondo, Patilan, Pantukan, Davao de Oro.
Immediately, the BFP Pantukan Fire Station responded to the reported
incident.

2. The incident involved an LPG explosion caused by the accumulation of

gas leak in an enclosed space combined with an ignition source. Two
people were wounded, Dorita Sakitan y Talahib, 85 years old, female,
married, Senior citizen, owner of the burned house and Tonio Luna y
Mansa, 48 years old, male, married, the one who saved the latter.

3. The burning house is owned by Mr. & Mrs. Talahib. At around 2:00 pm,
Belle Santos y Talahib, 10 years old female together with her
grandmother Dorita Sakitan y Talahib, 85 years old, female, were
cooking in the kitchen using LPG tank. Later, Dorita Sakitan y Talahib
noticed that the tank sounded unusual, seconds after a small fire
emerged. Belle Santos y Talahib ran for help while her grandmother
tried to stop the fire. One Tonio Luna y Mansa their neighbor, 48 years
old, male, helped in putting the fire but it was too late and they decided
to get out. According to their neighbors, while the fire is getting huge,
Dorita Sakitan y Talahib rushed to their burning kitchen hoping to save
her money worth of 100,000.00 but she was stop by Tonio Luna y
Mansa. Dorita Sakitan y Talahib suffered some burns and Tonio Luna y
Mansa with first degree burn to second degree burn. They were
brought to the hospital.

Memorandum from Barangay Captain Marcelo O. Poso of
Kingking, Pantukan, Davao de Oro

1. RATIONALE:

This pertains to the information received by this office regarding the alleged
robbery in HB1 Pharmacy Pantukan Branch in Pantukan, Davao de Oro at
about 7:00 pm on August 27, 2021, perpetrated allegedly by a fraternity
named “KAPPA REGINE x5”, a group of male teenagers. The group was
alleged of taking the money worth of 500,000.00 from the store’s vault and a
serious physical injuries to the two guards and slight physical injury to one
customer.

2. ACTION TAKEN:

Upon the received of the information through a memorandum from Barangay

Captain Marcelo O. Poso of Kingking, Pantukan, Davao de Oro regarding the
taking of money worth of 500,000.00 from the store’s vault and a serious
physical injuries to the two guards and slight physical injury to one customer,
personnel from Pantukan Municipal Police Station led by SPO2 Jerry P. Tore,
immediately responded/proceeded HB1 Pharmacy Pantukan Branch and
conducted initial inspection/investigation thereat.

Investigation disclosed that Five Hundred Thousand Pesos (P500, 000.00)

were taken by KAPPA REGINE x5 with serious physical injury and slight
physical injury.

Appropriate case against the suspects is being prepared by Pantukan

Municipal Police Station in coordination with HB1 Pharmacy Pantukan Branch
for filing in court.

Freshly Baked Bakery (located in same place) to buy
some bread. A riding in tandem stops in front of the
bakery and draws a pistol and point it to Oro. The victim
 Reynaldo Agusan y Oro, was pronounced dead on
arrival.

5. Why? The boy dialled the 911 emergency telephone numbers to see if
someone will actually answer his call.

6. How? The 10 years old boy was holding his phone while surfing in YouTube
until he saw a video showing the 911 hotline numbers. After he finished
watching, he went to hold down the power button and he saw an option for
Emergency Mode. He tapped it and it brought up five options: Flashlight,
Emergency, Share My Location, Phone and Internet. Below those options,
there will be a button for Emergency Call. The boy tapped the emergency
button and automatically called 911.

STATEMENT OF WITNESSESS:

Ninio Gura y Dora, 56 years old, married, male, resident in the area and owns a
vulcanizing shop was looking at the highway when the incident happened.
According to him, when the driver of Duke motorcycle overtakes the RS driver, he
moves in zigzag that cause to the both of them to crash.

POLICE DESCRIPTION OF ACCIDENT:

The drivers are both heading to Pantukan when Kenjie Doque y Mose (Duke
Skygo 150) did overtake Jeffry Tulip y Petals (RS 150 R V1 REPSOL). The
overtaking, speed of the vehicle (motorcycle) and human error caused the
accident.

DRIVER STATEMENT:
1. Jeffry Tulip y Petals, 49 years old, male, married, and a resident of Matiao,
Pantukan, Davao de Oro. According to him, he was heading to Pantukan
when he noticed a driver overtakes him. He was in shocked when he saw
the driver crashed and forgot to hold the brake.
2. Kenjie Doque y Mose, 52 years old, male, married, and a resident of
Cebulida, Kingking, Pantukan, Davao de Oro. According to him, he
overtakes because he received an urgent call from home. He also added
that there is something wrong in his motorcycle.

1. Memo from the Office of Pantukan Police Srarion dated August 26,
2021 an initial investigation report on the shooting incident in
Tambongon, Pantukan, Davao de Oro at about 8:00 pm.

II MATTERS TO BE INVESTIGATED

3. To determine the surrounding circumstances of the shooting

incident that killed Reynaldo Agusan y Oro.
4. To possibly identify the suspects that will lead to their apprehension
and possible prosecution.

III FACTS OF THE CASE

5. At around 8:00 pm in the evening of August 26, 2021, Reynaldo

Agusan y Oro, 30 years old, single, a resident of Tambongon,
Pantukan, Davao de Oro came to Freshly Baked Bakery (located in
same place) to buy some bread. A riding in tandem stops in front of
the bakery and draws a pistol and point it to Oro. The victim
Reynaldo Agusan y Oro, was shot once at the back of his left
shoulder but he still managed to run away. The riding in tandem
chased Oro but failed when the victim go through the houses. The
victim was immediately rushed to Compostela Valley Pantukan
Hospital (CVPH) however, Reynaldo Agusan y Oro is pronounced
dead on arrival.

IV INVESTIGATION/FINDINGS

6. The initial investigation conducted by the investigation team of

Pantukan Municipal Police Station at Freshly Baked Bakery located
 in Tambongon, Pantukan, Davao de Oro shows that the riding in
tandem keep on stopping by in front of the establishment. Allegedly
waiting for Reynaldo Agusan y Oro. The case it still under further
investigation.

V CONCLUSION

7. That on August 26, 2021 at about 8:00 pm, Reynaldo Agusan y

Oro, 30 years old, single, a resident of Tambongon, Pantukan,
Davao de Oro was killed by a riding in tandem in front of Freshly
Baked Bakery (located in the same place). The incident is under
further investigation.

VI RECOMMENDATION/DISPOSITION

8. The shooting incident that happened in Tambongon, Pantukan,

Davao de Oro on August 26, 2021 at about 8:00 pm is under further
investigation.
