LESSON 1: Kinds Of Report
What I Know
Copy the list of items below in your notebook. Identify which one is familiar to you,
you may have encountered or have tried to make it before. Put a check mark inside
the box before the item. You can check as many familiar items.

ACTIVITY 2
Identify the following reports reports and describe them according to purpose, and
structure. Write your answers in your notebook.
1. SCHOOL FORM 9 (Report Card)
PURPOSE: Used to automatically output two important records of student namely
the School Form 9(SF9) or Learner’s Report Card.
STRUCTURE: Include practical information such as transportation options and
school contact information. Design includes the text and graphics used to
communicate information as well as degree of interactivity that users experience.

2. BOOK REPORT SAMPLE PAPER


PURPOSE: An objective summary of the main ideas and arguments that the book’s
author has presented. The purpose of the report is to give enough information to
help decide whether the book will be of use or interest to any potential readers.
STRUCTURE: Include basic bibliographical information about the work, a summary
of a narrative and setting, main elements of the book, and a theme statement
summing the events.

ADDITIONAL ACTIVITIES
I noticed the different kinds of report and their purpose to enlighten the readers.
A question I have is does all the kinds of report have the same routine before and
after it’s release?
I’m not sure if I can try all the kinds of report I still have to research some good topics
and more books to read in able to understand it more.
I realized it’s importance to the readers and the writers.

LESSON 3: Conducting the surveys/Experiments/Observation

ACTIVITY 1. HOW TO DO IT
First, the researcher must prepare the questionnaires then determine the number of
participants and seek permission to the participants to conduct the survey. Second,
administer the questionnaires the participants answered.
Third, gathered the data and carefully check every questionnaires if it’s answer all
the questions.

Before deciding to conduct the survey, list down the methods discussed above and
reasons why you will use and why you will not use each. State it in your own words

ACTIVITY 3: Compare and Contrast


CONTRAST
Observational Study
The sample population being studied is measured, or surveyed, as it is. The
researcher observes the subjects and measures variables, but does not influence
the population in any way or attempt to intervene in the study. There is no
manipulation by the researcher. Instead, data is simply gathered and correlations are
investigated. Since observational studies do not control any variable, the results can
only allow the researcher to claim association, not causation (not a cause-and-effect
conclusion). Surveys are one form of an observational study, since the researchers
do not influence the outcomes.
Experimental Study
Has the researcher purposely attempting to influence the results. The goal is to
determine what effect a particular treatment has on the outcome. Researchers take
measurements or surveys of the sample population. The researchers then
manipulate the sample population in some manner. After the manipulation, the
researchers re-measure, or re-survey, using the same procedures to determine if the
manipulation possibly changed the measurements. Since variables are controlled in
a designed experiment, the results allow the researcher to claim causation (a cause-
and-effect conclusion).

I collected all the survey questionnaires and check all of it before thanking the
respondents participation to make sure no unanswered questions.I summarized the
gathered data through pie chart to be easily seen the difference of it.

Based on the data above, create two (2) generalizations. Draft one paragraph for a
discussion of your findings in each set of data. Tell what you have seen and learned
from the data. Analyze and give objective conclusions. Write your answers in your
notebook.
The Second data I used is the Pie chart to see more clearly the sites that Teenagers
visits everyday. As a student and at the same time teenager, I can totally relate to
those teenagers who visits more on Facebook than answering or searching an
answer to Google. They can express more on their social media than in school, they
can show off more on social media than participating and joining school activities.
Distance Education is being promoted to become one of the learning modalities and
to my perspective teenagers are being tempted to visit Facebook than to answer
their modules. Since teenagers are more active on social media, parents guidance is
a must.

Gather the collected information after conducting the survey. Summarize your
findings in an organized way. Then, add a paragraph of discussion after your

The table above shows the respondents majority answers. To my conclusion based
on the gathered there are more families who doesn’t do any hobby or activities with
their family, instead they prefer working because of the situation nowadays.
The second figure above shows the majority of other fun activities they come up with
to enjoy it with their family like sport’s day, eating lunch outside, travel and more.

ACTIVITY 4. REPORT IT
COVID-19 cases are still a threat to every human with it’s rapid growth infecting
people all around the globe. In the late 2019 a new virus was born and still
diminishing human race in the present year 2021, millions of people have been
suffering equivalent to thousand of deaths everyday. All countries have been
experiencing the same situation, the death toll we’ve seen to every country are really
hasn’t leave any information platform reporting new cases and number of deaths. As
the year passed COVID-19 is also evolving making it more dangerous and more
contagious . United States holds the highest number of COVID-19 cases in the world
followed by India and Brazil, as I said earlier COVID-19 evolved also into new variant
found in Europe specifically in the United Kingdom, calling it a new UK variant and
still spreading in the present day. Southeast Asian countries also had a fair share to
the growing number of COVID-19 cases, Indonesia takes over the Philippines to
have the highest number of cases since months of being quarantined. As of today,
scientist have been working hard finding a cure to end the virus waves of death
through making a vaccine and to lessen the growth of mortality rate everyday.

I noticed that there are also another method of data gathering because I don’t
usually recognize some mail type data gathering.
A question I have is can I still do some mail type nowadays?
I wonder why people don’t respond to mail type questionnaires.
It seems like technology take over time when it come to data gathering.
