to which the indicator meets the criteria. Put a tick [/] mark in the appropriate column
corresponding to the criterion being evaluated. Also, give the appropriate rating with 4
being Very Satisfactory (VS); 3 - Satisfactory (S); 2 - Poor; and 1 – Needs Improvement
(NI).

If a rating is below 4, please write your comments and/or justifications on each evaluation
criterion. Write Not Applicable (NA) if an evaluation criterion doesn’t apply or of no value.
Attach extra sheets if necessary.

For the Reading Program to PASS, it must at least obtain a general rating of
SATISFACTORY, upon incorporation/integration of the recommendations on findings or

in at least one of the factors in this rating sheet should be revisited and reviewed by the higher
authorities.
