PART IV. INSTRUCTIONS

1.- This form must be delivered to the H&S department at the start of works by each company that requests authorisation to
have vehicles.
2.- The required documents for each vehicle must be attached to this form, including the corresponding use authorizations and
driving licences.
3.-Fields highlighted in pink must be filled in by the H&S Department after reviewing the documents corresponding to each
vehicle.
4.- It must be delivered together with the corresponding parking badges and authorisation stickers that allow them to drive
around the worksite.
5.- Access Control shall sign the original documents and keep a copy.
6.- If a company for which vehicles have already been authorised wishes to have more vehicles authorised, it must only fill in
and sign table V of this form.
7.- Once this form is duly filled in, it must be filed in the H&S archives.
