This request pertains to our contract for 22LO-0030: Construction of Concrete Road - Panaga-
Pandaitan(SitioTibungol)-Paradise Embak-Jct. Mabuhay-Pañalum Road-Package 1, Davao City. Our
scope includes the supply of concrete, labor & equipment.

Please be informed that there are changes in the material specification where we were directed to use
admixture in our concrete mix. Admixture is not included in our quotation since we were not informed
about this and this is our first Portland Cement Concrete Pavement project under contract with your
company.

In our meeting with your construction & material quality control department heads we come up to an
idea to request for change order.

In this regard, we are respectfully requesting for Variation Order on this matter.

Your utmost attention and favorable consideration will be highly appreciated.
