A RESOLUTION SEEKING TO OPEN A SAVINGS ACCOUNT

WITH COOPERATIVE BANK OF NEGROS ORIENTAL
BAYAWAN BRANCH AS FUND DEPOSITORY
OF THE ASSOCIATION

WHEREAS, the Tribu Lumad Bukidnon-Magahat Association of Barangay Narra, Bayawan City is an organization of
Indigenous People duly registered with the Department of Labor and Employment (DOLE) under Registration No. 4839
dated February 23, 2012 and accredited to as co-partner under DOLE-DILP.

WHEREAS, the said Association has its members who are registered beneficiaries of Modified Conditional Cash
Transfer for Indigenous People in Geographically Isolated and Disadvantaged Area (MCCT-IP in GIDA) under Pantawid
Pamilyang Pilipino Program of DSWD and created a Committee on Egg Layer Production Project.

WHEREAS, the Committee on Egg Layer Production Project has its members, and the key officers who are also
the bank signatories in relation to the implementation of said project.

WHEREAS, the Committee on Egg Layer Production Project is planning to open a savings account for their
livelihood project (Livelihood Enhancement through Egg Layer Production and Management Training Cum Provision of
Starter Kit) with an account name TRIBU LUMAD BUKIDNON MAGAHAT (MCCT-IP in GIDA Egg Layer Production Project).

Whereas, it is deemed fit and proper to deposit and withdraw funds of the association through a livelihood
committee to a reputable bank as official depository bank.

WHEREAS, the association - TRIBU LUMAD BUKIDNON MAGAHAT-MCCT IP IN GIDA (EGG LAYER PRODUCTION
PROJECT) shall open a savings account at COOPERATIVE BANK OF NEGROS ORIENTAL BAYAWAN BRANCH as
funds depository.

THEREFORE, RESOLVED AS IT IS HEREBY RESOLVED, to open a savings account with the COOPERATIVE
BANK OF NEGROS ORIENTAL BAYAWAN BRANCH and authorizing the four (4) signatories below to sign any
withdrawal from the said deposit savings account, viz:

RESOLVED FURTHER, to provide the COOPERATIVE BANK OF NEGROS ORIENTAL BAYAWAN BRANCH a
copy of the Resolution for its information.
