JAGNA BRANCH, JAGNA, BOHOL UNDER THE ACCOUNT NAME OF DSWD
KALAHI CIDSS: NCDDP-AF/BRGY. CAWAYANAN, MABINI FROM PIO F.
FUDERANAN – BARANGAY CHAIRMAN TO LIGAYA P. ESTRELLA - BARANGAY
DEVELOPMENT COUNCIL – TECHNICAL WORKING GROUP (BDC – TWG) HEAD,
TO MAINTAIN VENCITA B. MACA – BARANGAY TREASURER AND AUTHORIZING
FLORA S. COLINA – AREA COORDINATOR AS THE CO - SIGNATORIES THEREOF.

WHEREAS, Barangay Cawayanan, Mabini, Bohol is one of the recipients of KALAHI

CIDSS: NCDDP Additional Financing being implemented by the Department of Social
Welfare and Development (DSWD) and funded by World Bank;

WHEREAS, the disbursement or any transaction in relation to the account during

sub-project implementation, the Barangay Treasurer, BDC-TWG Head and the Area
Coordinator are authorized signatories of the said sub-project;

WHEREAS, the Barangay Council of Cawayanan agree that Land Bank of the
Philippines (LBP), Jagna Branch is the official depository bank of DSWD KALAHI
CIDSS: NCDDP-AF/BRGY. CAWAYANAN, MABINI Account;

WHEREAS, this resolution remains effective and subsisting and has not been
amended, revoked or superseded;

WHEREAS, the Barangay Council approved and authorized the signatories of

Community Bank Account DSWD KALAHI CIDSS: NCDDP-AF/BRGY. CAWAYANAN,
MABINI Account;

NOW THEREFORE, on mass motion and duly seconded by the same;

RESOLVED, AS IT HEREBY RESOLVED, to amend and/change of the official bank

signatories at Landbank of the Philippines (LBP), Jagna Branch, Jagna, Bohol under
the account name of DSWD KALAHI CIDSS: NCDDP-AF/BRGY. CAWAYANAN,
MABINI from Pio F. Fuderanan – Barangay Chairman to Ligaya P. Estrella - Barangay
Development Council – Technical Working Group (BDC – TWG) Head to maintain
Vencita B. Maca – Barangay Treasurer and authorizing Flora S. Colina – Area
Coordinator as the co - signatories thereof;

RESOLVED FURTHER, that copy of resolution be furnished to DSWD-Field Office VII-

Regional Program Management Office (RPMO), Area Coordinating Team (ACT),
Municipal Coordinating Team (MCT) and Land Bank of the Philppines (LBP), Jagna
Branch, Jagna, Bohol for appropriate action;

Unanimously adopted and approved this 16th day of January 2023 at Barangay
Cawayanan, Mabini, Bohol.
