Observation notes
MD X NA Earth works The pit was excavated to a required depth as per the
ok Sub-structure drawings. The pit wall was built in header bond and
ok Cross beam, columns and plinth well finished. The columns and cross beam was put
walling in place as per the drawings and BOQs. The top
slab was reinforced with BRC as specified. The
splash apron and ramps were put in place as per the
drawing.
ok Super structure The super structure especially walling was built in
stretcher bond with hoop iron reinforced at alternate
courses.
ok Finishes The wall was well plastered, painted and ready for
use
Ok Wall painting Wall painting down in conformity to requirement of

Ok Roof Provided as required in the BOQ
ok Doors and ventilators Well installed, painted and accessories provided
Findings
Sub-structure.
The pit was excavated to a depth of 3500mm from the ground level as required by the Drawings.
The bed of the excavation was blinded with hardcore and casted with concrete of C15 (1:3:6).
The wall of the pit was built in header bond with mortar of 1:3, the internal wall was plastered,
column casted at the corner points, cross beam put in place as per the specifications.
The top slab was reinforced with BRC and casted with C15 Concrete (1:3:6). The man hole and
its cover was provided for.

RECOMMENDATIONS
The contractor did satisfactory work as witnessed by the serviceability and structural
checks done on the structure.
I do recommend payments be done for the work done as per this report.
