I hereby certify that I am aware of the safety rules and the safety items that I must be wearing while I am
conducting the experiments in the lab. I realize that many accidents are caused by carelessness and being
in a hurry. I will come to the lab prepared to be responsible so that the safety and welfare of me and
others is not jeopardized.

the instructor and I agree to follow these and any other rule instructed in the lab.
