1.0 Confined Space 1.1Asphyxiating/Toxic/Flammable 1.1.1 A CS Entry Supervisor must be assigned. Operation/ Maintenance
atmospheres 1.1.2 Develop a CS Entry Plan /contractors
1.1.4 Follow precautions highlighted in the CS
Entry Permit.
1.1.5 Conduct frequent gas testing
1.1.6 Provide CS training to personnel entering.
1.1.7 Barricade the area and add warning signs.
1.1.8 Assign a standby man.
1.1.9 Keep gas log record
1.1.10 Keep entry log record
1.1.11 Precautions shall be taken to ensure that
air contaminants from adjacent
processing or chemical handling cannot
enter the confined space.
1.1.12 Atmospheric gas tests shall only be
conducted by a SA-certified gas tester.
1.2.1 Blinding or disconnecting of piping shall be
the only acceptable method of isolation
2.0 Excavation/Trenching 2.1.Contact U/G utilities 2.1.1 Excavation Safety Checklist (see CSM II-1,
Excavations and Shoring). Operation/ Maintenance
2.1.2 Develop an excavation plan by a degreed /contractors
engineer when applicable.
2.1.3 Identify all utilities and lines and notify
appropriate parties before beginning
any digging or cleanup work.
2.1.4 Mechanical excavation are not allowed
inside process areas, unless approved by
Division Head.

2.2 Hazardous atmosphere 2.2.1 Entry into excavations 1.2 m (4 ft) and
deeper will require a CSE permit.
2.2.2 Adequate ventilation must be provided.
2.2.3 Standby men shall monitor activities inside
and outside the confined space to
determine if it is safe for entrants to
enter and/or remain inside the space.
2.2.4 Position scaffold bases at least 1.5 times the
depth of an excavation away from the
edges of the excavation (including
trenches).
2.2.4 Inspect the excavation using the Daily
Excavation Checklist.
2.3 Excavation Collapse/Injury
2.3.1 Design excavations using an excavation
competent person.
2.3.2 Use proper benching, shoring, sloping
2.3.3 Do not use mechanical excavators within 3
m (10 ft) of any underground pipes,
cables, or other obstructions.
2.3.4 Position scaffold bases at least 1.5 times the
depth of an excavation away from the
edges of the excavation (including
trenches).
2.3.5 Erect barricades at a safe distance and
provide warning lighting for darkness.
2.3.5 Stop work in excavation when water
seepage present until conditions are
clear.
2.3.6 Provide safe access to excavation (ladder).
2.3.7 Personnel working inside excavation to
keep safe distance from each other..
3.0 Cable laying 3.1 Presonal Injury 3.1.1 Personnel shall wear Personal Protective
Equipment such as Hardhat, Eye
