SUBJECT : COST PROPOSAL FOR REVIEW AND SIGN & SEAL OF ELECTRICAL PLAN

Dear Sir,

I am pleased to submit my cost proposal for the particular project above. Listed below are
the scope of works.

SCOPE OF WORKS

A. Electrical System
1. To review and check/vetting plan of “For Permit” Plans.
2. Re-checking should there be corrections in the design calculation, if any. The
designer is accountable for the revision and CAD reproduction.
3. Printing of “For Permit” plans. (see item B)
4. To sign and seal of “For Permit” Plans.
5. Delivery and handling.

B. Submittals
1. 12 sets of Blueprint, Signed & sealed For Permit Plan.
2. 3 sets of PTR & PRC ID copy (signed & sealed).
3. Technical Specifications.

Excluded and Limitation

1. Permit Facilitation.
2. Preparation & execution of As-built plan.
3. Construction Coordination meeting, as needed only (with design concern)
4. Special meetings, site visits & project coordination meetings requested by client
shall be charged separately at a rate of Php 5,000 per day as Professional Fee to
include hotel accommodation, air fare and other transportation expense.

C. Professional Fees

Service Fee shall be: EIGHTY THOUSAND & 00/100 PESOS ONLY

D. Duration

7 days upon GGEC’s clearance/advise to print frozen plans.

 E. Terms of Payment

Full payment upon issuance of NTP
