SUBJECT: Cost Proposal for the requirement of DENR for Generator and Boiler

Dear Sir,

We are submitting our cost proposal for the requirement of DENR for Generator and Boiler
located in 0145 P. Gregorio st., Lingunan, Valenzuela City, lump sum amount of One hundred
twenty thousand and 00/100 (PhP 120,000.00) Pesos only, Vat and all net taxes are
Excluded.

I. Scope of Works

A. As-built drawing of Generator and Boiler. (Signed and Sealed)

B. Engineering Report.

C. Engineer on record, signing of blueprint of plans.

D. Estimated Emission of Generator

E. Mechanical Permit form for City Hall - Valenzuela (Signed and Sealed)

F. Technical Specification of Generator and Boiler (Signed and sealed)

G. Bill of Quantities / Bill of Materials as required by DENR (Signed and Sealed)

H. Printing of Documents 1 copies only, other copy shall be providing by the client

II. Deliverables
-As stated in scope of works and to be finished 3 days upon receiving the down-payment. In case
all the deliverables are exceed in 3 days a 20% deduction from the proposal shall be imposed.
III. Payment Terms
- 50% Down-payment and upon signing of contract

- 50% Upon signed and sealed

Notes and Conditions:

- Any extra copy of plotting and printing of all drawing sheets and documents.

- Attendance/meeting during construction on the project site and appearance requested by

the Government/building official or Project management team which serves as a Technical
Consultant; if required/needed an additional Php 10,000.00/Diem.

- All delivery/transportation cost of the drawings/technical documents shall be charged by the

client.

- Client shall provide a sketch or floor plan for the location of the boiler and generator.

- Scope of works are based on the DENR requirements.

This proposal is valid for 15 Calendar days only.
