1. INTODUCTION

QSAFE Observer of the Month is a tool to motivate employee’s involvement and participation in
the QSAFE process with the desired target of improvement in observation skills.

2. SCOPE

This process applies to all QCHEM, QII, RLOC and Salam Tower employees and base load
permanent contractors at the plant site and headquarters Salam Tower.

3. DEFINITIONS

QSAFE is a behavior based safety process coordinated by Q-Chem employees to focus on safe
and At-Risk behaviors and conditions in the workplace.

Safe Behaviours is an observed behaviour that has no potential negative consequence and
which helps in building a safe working culture in the organization and increases the likelihood of
the same in future.

At-Risk behavior is an observed behaviour that has potential negative consequence to cause
injury or illness if continued or not corrected.

Clean Sheet is an observation sheet submitted, with out any Safe and / or At-Risk comments.

QIT - QSAFE Improvement Team

SC - Steering Committee

Department’s Observer of the Month is best observer selected by QIT group from their
respective department / group.

Q-Chem’s Observer Of the month (Plant Wide – Mesaieed + RLOC + QCII + Salam Tower)
is best observer selected by SC from all nomination of Department’s Observer of the Month.

OOM Shortlisting Team is a sub-team of atleast 4 current SC / QIT members which will
evaluate and shortlist best Q-Chem’s Observer Of the Month comment based on the stipulated
ranking criteria.

4. GUIDELINES

4.1. QSAFE observer of the month selection:

4.1.1. Department’s Observer of the Month:

Each QSAFE Improvement Team will select one best observer from their respective group/
department per month based on Quality of observation applying following criteria:

• Observer has done minimum 4 observations.


• Out of 4 at least 1 should be coached observation. (Coached observation is
observation done by employee with QIT member).
• Number of At-Risk behaviours observed should be equal or greater than 50% of total
number of observations. (This will cover the clean sheets).
• Percenatage of behaviours marked (# of behaviours observed / # of total behaviours)
should be within 40%-70% range (This will eliminate the marked behaviours which
are not really applicable or not observed).
• At-Risk count per sheet (# of Risk / # of Sheet) should be within 0.5 to 2.0 ranges.
(This will eliminate abnormal high At-Risk count)
• Observer need to pass through all above mentioned criteria to become eligible
candidate for observer of month. (Except coached observation, rest of all filtration can
be done with help of a macro file )
• QIT will select best observer from all eligible candidate by analyzing contents of the
sheets with regard to clarity of comments; effectiveness in identifying the At-Risk
behaviour and potential consequences; impact of the solutions to correct At-Risk and
Safe Behaviour comments.
• Selected Candidate will become observer of the month of respective QIT group.

4.1.2. Q-Chem’s Observer Of the month (Plant Wide – Mesaieed + RLOC + QCII):

• Each QSAFE Improvement Team will nominate the best comment for Q-Chem’s
Observer of the Month competition.
• The OOM Shortlisting Team will select best observer from all eligible candidates by
analyzing contents of the sheets with regard to clarity of comments; effectiveness in
identifying the At-Risk behaviour and potential consequences; severity of At-Risk
behaviour corrected and impact of the solutions to correct At-Risk and Safe
Behaviour comments by using following guidelines:

 Each 5 elements of At-Risk comments ( WHILE, WAS, WHY, SUGGESTION, TRY /


AGREEMENT) will be ranked between 1 to 5 in ascending quality manner, as per the
guidelines referred at 4.3
 Sum of all 5 ranking would be considered as Basic Points.
 10 % of the basic points will be credited to basic points for ‘Special Safe Comments’.
Generic safe comments such as ‘found wearing all PPE’ will not be considered as
‘Specific safe comments’.
 10 % of the basic points will be deducted from basic points if wrong selection of At-
Risk behaviour has been made in writing the observation comments.
 Sum of Basic Points, Credit Point and Debit point will make the Final Score.
 The comment with highest Final Score will be selected as Q-Chem’s Best Observer
of the month.

• SC team will review and authenticate selection before announcement during monthly
SC meeting.

4.2. Eligibility Criteria:

• All trained observers (Non QSAFE Improvement Team members)

4.3. Quality Observation Comment:

A quality observation comment should be able to draw the picture while being read. It should
have following details mentioned in respective sequence:

• While : Condition identification for At-Risk Behaviour


(Observer) Specific time and activity when At-Risk behaviour observed

• Was : Hazard Identification and Consequences


(Observer) At-Risk behaviour / condition observered and the potential
consequences

• Why: Barrier Identification OR Root Cause of At-Risk behaviour


(Observee) Let observee identify the root cause of At-Risk behaviour
Don’t get satisfied with general answers like ‘FORGOT’
Dig more until find out REAL BARRIER

• Suggestion: Most feasible way to do thing safely


(Observee) Product of Discussion between Observer and Observee
Motivate observee to provide solution
If observee fails to do so, then provide appropriate solution
Derive necessary measures to continue job safely
Identify FOLLOW UP action if required as part of long term solution

• Agreement Try: Observee’s agreement / action taken to correct behaviour


(Both) Observer should witness the corrected behaviour if possible

4.4. Observer of the Month Recognition:

4.4.1. Observer of the Month - Plant Wide:

• QSAFE Certificate with the frame - signed by Q-Chem management representative


• A picture with the Plant/Department Manager and / or QSAFE Management Sponsor
• Gift item with QSAFE Mascot
• All recognitions will be awarded during the Q-Chem’s monthly Manager’s Safety
Communication Meeting.

4.4.2. Department’s Observer of the Month:

• Recognition with prizes and certificate.

5. RESPONSIBILITIES

QIT:
• Each QIT is responsible to select best observer from their respective group / department
on a monthly basis.
• QIT is responsible to select and nominate the one best Safe Comment and best At-Risk
comment of observer of the month for Q-Chem’s Observer Of the Month competition.

SC:
• SC member is responsible to submit the best comments of departmental Observer of the
Month for Q-Chem’s Observer Of the month competition by posting it on QSAFE intranet
page.
• Comments must be submitted two (2) days before the Steering Committee (SC) meeting
(every second Monday of the month).

OOM Shortlisting Team


• OOM Shortlisting Team comprise at least 4 current volunteered SC / QIT members which
is formed by QSAFE facilitator.
• The members of sub-team will be rotated on quarterly basis.
• The sub-team will select the best comments out of all nominations for plant wide
observer of the month prior to SC meeting and present to the SC team for final review
during SC meeting on a monthly basis.
• SC team will review and authenticate the final selection before announcement.

QSAFE Facilitator:
• QSAFE Facilitator will compile all nomination and make a plant wide announcement for
Observer Of the Month winners.
• QSAFE Facilitator will arrange the recognition for observer of the month winners.

Plant Leadership Team:


• Plant leader ship team will recognise the observer of the month in leadership safety
meeting.
