3) What are the safety protocols that we’ve already discussed were not followed by the worker?
Elaborate your answer.
-The accident mainly happened because of the lack of safety equipment that would prevent the
worker from falling. Also, if the worker at least had a safety hat, shoes, or any type of PPE he
would’ve still been injured but there is a bigger chance that he would have survived. However, the
important part must be that all safety protocols such as the safety barricade and personal protection
system must be followed. Finally, a safety officer must be present to oversee the worker/s such that
when something happens, a first aid procedure will be done immediately.

4) Who was at fault in this accident? Justify your answer.

-The administration mainly holds the responsibility for the accident that occurred, as they failed to
comply with the equipment needed to proceed with the works safely. They also failed to do a safety
inspection for the hazard identification. Moreover, a safety officer was not present when the
workers were doing their duty.
