 Scaffolding collapse.  The ladder should be fixed for safe access. 1:4 ratio or 70 degrees.

 Electrocution hazard  A 12 mm thickness Lifeline should be installed for FBH anchoring.

 A proper platform should be provided for working at height. Ensure

top rail, mid rail, toe guard and other required components as per IS
code.
 If required Safety net should be provided under working platform.

 Toolbox talks shall be conducted for the workmen.
 Ensure all manhole and floor openings have covered well with adequate
proper signage.
 Ensure used height working platform and scaffold have certified and
inspected by competent person with the tag.
 Ensure all power supply has taken through RCCB/ELCB of 30mA
sensitivity.
 A dedicated licensed electrician should stay available at site during all
working hours.
 Follow good posture of work during work.
