Task 1: Fire arrangements

Question 1 (a)
Fire arrangements include implementations, controlling, monitoring, and reviewing of fire
safety standards in the workplace. Below are the positive points to note about fire
arrangements at MSL:
1. There is a manual call point and alarm recognition system in place to timely activate
fire alarm and make workers to timely respond to fire situation and evacuate the building
during fire emergency.
2. There is an induction training that cover site rules and fire arrangements in the
workplace to help the various workers to be familiar with the fire hazards and risk
inherent in the workplace.
3. There is also fire notices at various points around the warehouse and along call points
near the doors as demonstrated in the scenario.
4. There is also a designated assembly point for head count to enable people to know if
the workers are complete or if some are still remaining in the building.
5. There is a proper designed assembly point as mentioned in the scenario for moving
the worker as well as visitors to that point in case of any emergency situation.

Question 1 (b)
1. There is no regular fire drill as the last fire drill was last carried out in 2018 as
seen in the scenario. This will make workers to be confuse and not to be familiar
any new technique included in the fire drill process. And they may not know what
to do in the event of a real fire outbreak.
2. From the scenario the MD said “Everyone has been told that the assembly point
is in the car park at the back of the building”. This suggests that workers were
only given information on where the assembly point is. However, relevant
information on how to access the assembly point was not given.
3. There is no fire risk assessment in place because the HSM do not know what to
include in the fire risk assessment.
4. Having just one muster point and for the fact that there is chaos suggests that the
one muster point may not be sufficient for the workers
5. The muster point being at the car park as demonstrated in the scenario may pose
another risk in the workplace as there might be collision between vehicles or
between a vehicle and a worker.
6. Workers have not been given adequate training and information about how to
organize themselves at the muster point and that may be what resulted to chaos
on the assembly point.
7. There is no training provided to workers about firefighting, as training enables will
enable the workers to know the right approach to use firefighting equipment in the
event of any fire outbreak.
8. A designated muster point has been provided at the car park but people are not
assembles there. This may probably be due to the fact that there is no appointed
person to escort them to the actual muster point at the car park.
9. The MD does not see health and safety as a priority as he believes that fire drill
will disrupt the business.
10. People were seen taking up role of fire marshal even without authorization from
the organization.

Fire drills are organized actions practiced in workplace and are taken in the event of any
fire outbreak. The following given below are the importance of fire drill:
1. To identify weak sport: to know what aspect of the system is faulty or to know the
effectiveness of the system
2. Frequent fire drills will help to assess performance and effectiveness of fire safety
measures at MSL.
3. To achieve coordinated and timely response in the event of any fire emergency.
4. To test the alarm system if they are really functional.
5. To fulfil legal requirement.

Task 2: Managing risks for those working from home

Question 2
1. The Sales Manager (SM) noticed that more than one of the sales team workers
has poor posture in their chair during online meetings thereby exposing them to
musculoskeletal disorder.
2. There is guidance document which includes advice for workstation assessment
for long-term working from home.
3. There is a regular 1-2-1’s online meeting with the individual team members and
twice-weekly sales team meetings to enable health and safety issues to be
addressed.
4. There is regular update from each member on what they are doing, and among
other things, the wellbeing of the team is discussed to improve work morale,
safety in workplace and also to boost sales.
5. There is also opportunity for workers to chart with one another as this will help
address any issue of lone working since workers will bonded with one another.
6. There should be provision of health surveillance programs for workers. This will
help in detecting ill-health effect at an early stage to enable MSL to provide better
control measures before it will get worse.
7. There should be provision of work support programs for workers to do their work
effectively.
8. Workers should be given advice to carry out exercise at home to enable them
stay fit.
9. Long term working from home should also include good nutrition for workers
health and safety.
10. Ergonomic risk assessment should be carried out and adequate training should
be provided for all workers at MSL.

Task 3: Suitability of the generic risk assessment / risk assessments

Question 3
1. The MD gave me a directive to carry out a full review of their health and safety
following the recent and previous accidents because of the enforcement visit.
2. The MD wants me to compromise by producing a report demonstrating that the
management of health and safety at MSL has nothing to do with the accidents
and that both accident were due to workers error.
3. The directors believed that there was no risk if common sense is used. Their

perception to risk is low. However there are visible some risk like DSE and stress
at MSL.
4. Directors are more interested in making money rather than putting control
measures in place to tackle health and safety issues.
5. Small forklift are put in place for restocking to ensure that the risk associated with
musculoskeletal disorder are eliminated.
6. Team leaders believe that assessment of risk is based on compliance to
instruction. However, risk can arise even if there is compliance to instruction. The
perception to risk of the organization is low.
7. There is no job procedures of working at MSL as there is there is no written job
description provide for new workers as demonstrated in the scenario.
8. There is no form of specific or generic fire risk assessment because the Health
and Safety Manager (HSM) do not have the requisite knowledge on what to
include in the fire risk assessment.
9. MSL have a specific risk assessment for the good-outwards warehouse rather
than generic. This shows that the risk assessment follows legal requirement. It is
suitable and sufficient.
10. There is no periodic review of the generic goods-outward warehouse risk
assessment as the last was done in 2018.
11. The last risk assessment was developed by the HSM alone rather than including
competent persons like the warehouse manager, team leaders and even workers.
12. The findings of the risk assessment were fed back to the workers by the team
leaders. This will give the workers a sense of ownership and inclusion.
13. I intended to review the risk assessment and this will offer the organization (MSL)
the opportunity to update their risk assessment.
14. The significant findings from the audit review indicate that many risks were not
assessed. As MSL tend to ignore many risk that are inherent in the workplace.
15. HSM analyzed documentation such as risk assessments, spoke to workers and
recorded their feedback in order to know the hazards and risk inherent in the
workplace.

Task 4: Monitoring and measuring (audits)

Question 4 (a) (i)


1. Lack of auditing experience: Though HSM have previously carried out safety
inspection, but do not have any experience about audit yet they went ahead to
carry out audit. Which is not a good safety practice.
2. According to the scenario, the HSM says they did the audit alone as they do not
have anyone to help them. This does not suggest a good safety practice as a
minimum of two persons is required for an audit to be carried out.
3. The HSM made the director to believe that they could independently carry out
audit even when they do not have the requisite skills and experience to carry out
an audit.
4. The audit was probably done without a plan and scope because the MD feel that
the HSM knew what they were doing meanwhile the HSM is not competent
enough to carry out audit
5. The HSM also supplied incorrect information by saying the directors were not

interested to see the findings from the audit and that they were too busy.

Question 4 (a) (ii)


1. The HSM analyzed documentation such as risk assessments
2. The HSM also consulted with workers by speaking with workers and recording
their feedback during audit process.
3. Also significant findings of the audits were presented in a report basically for
reference purpose.

Question 4 (b)
1. The directors did not assign competent persons to assist in the audit process.
2. directors does not show interest in the audit as they said they were too busy as
demonstrated in the scenario
3. The directors ought to have reviewed the plans and scope of the audit, however
they ignored it because they were not interested and were also convinced by the
HSM that they can handle the audit.

Task 5: Adequate first-aid provision

Question 5

The following consideration need to be made in a review for first aid needs to be realistic
and proportionate:
1. The size of the organization: The size of the organization by virtue of the
number of workers at MSL. MSL have 1200 workers, so the amount of first aid
provision should be proportionate to the number of workers in the workplace.
2. In the scenario, no professional first aider was appointed. Therefore, a
specialized first aider must be appointed in the workplace so that worker go run to
in the event of any accident in the workplace.
3. The type of workforce: The type of workforce should also be considered,
particularly the vulnerable people like new workers.
4. First aid need will depend on the type of work activities being carried out in the
workplace such as the use of Display Screen Equipment (DSE), distribution of
pharmaceutical products which will the use of vehicles for transportation and so
on.
5. Past experience like previous accident record should also be taking into
consideration. This should include the type of harm caused i.e. broken ankles and
also the location of the accident.
6. There should be proximity to assessing secondary health care system. In a
situation where external emergency medical services are not easily available,
more first aid provision may be need, for example first aid room.
7. There should be an arrangement of refresher trainings on first aid to personnel
and record should be made.
8. The hazard inherent in the workplace should be taking into consideration.
9. For first aid to the realistic and proportionate, it should be provided in line with

legal requirement.
10. We should also consider the suitability and sufficiency of the first aid equipment in
the workplace.
11. There should be a periodic review in the hiring criteria as first aid personnel
should be highly certified and experienced.
12. The location of the first aid equipment; if it is evenly distributed and accessible to
everyone should also be taking into consideration.
13. The work pattern and shift system should also be taken into consideration for first-
aid provision to be realistic and proportionate
14. There should be availability of sufficient number of competent first aider at MSL.
15. There should be provision of first aid for workers working outside the workplace
and those working from home as demonstrated in the scenario.

Task 6: Health and safety management roles and responsibilities

Question 6
1. It was the responsibility of the HSM to conduct a fire risk assessment. However,
they failed to do so because they do know what to include in the risk assessment.
2. It is the responsibility of the MD to carry out review on health and safety. The MD
fulfil this responsibility by appointing a competent person to ensure that this is
done.
3. It is the responsibility of senior management (HR manager) to give a clear job role
to new workers, however there are no clear instructions of job role at MSL.
4. It is the responsibility of top management, particularly the HSM to ensure that
there is periodic review and update of all risk assessment. However he failed as
the generic goods-outwards warehouse risk assessment was last reviewed in
2018.
5. The HSM is played a crucial role by providing first aid training for workers at MSL.
6. It is the responsibilities of the HSM to participate in consultation with workers,
however he failed as the last risk assessment was consulted by the HSM alone
without involving the workers.
7. It is also the responsibility of senior management to give equal priority to health
and safety. However, they did not as they feel that it is a disruption of business.
8. It is the responsibility of the MD to appoint a competent person in the workplace to
address health and safety issues. The MD fulfil this by appointing competent
persons who have already started Nebosh Diploma
9. It is the responsibilities of senior managers to take part in health and safety
activities such as inspections, health and safety audit and risk assessment.
However, they failed as they claim to be too busy for such activities.
10. Workers also failed in their responsibility in reporting all incident and accident as
the HSM doubts that all accidents and incidents are not recorded.

Task 7: Financial arguments to improve health and safety

The financial reason of health and safety will help the organization to avoid various direct
and indirect cost as depicted in the scenario of which some can be insured and many
uninsured. It is therefore pertinent to give dominant consideration to the health and safety
of workers due to the following financial justification:
1. To avoid cost due to staff turn-over as lots of workers will tend to leave the job
due to low priority be given to their wellbeing.
2. To avoid cost arising from investigation, fees for investigation and interruption of
work operations during the investigation.
3. To avoid cost arising from loss of workers morale several workers witnessed this
accident and have been traumatized by what they saw. This may affect
productivity.
4. To avoid cost arising from damaged reputation and brand of MSL which may
make them to lose the customers.
5. To avoid first aid and medical cost as workers that sustained broken ankle must
be given medical attention.
6. To avoid cost of conducting re-training and recruiting more new workers as a
result of high turn-over rate.
7. To also avoid fines that may be due to non-compliance of health and safety rules.
8. To also avoid fines due to compensation claims as injured worker may file a suit
against MSL.
9. Another financial justification is to avoid fines that may be initiated by the
enforcement officer. If heavy fine is levied on MSL, it may affect the running of the
organization.
10. If workers well-being as regards to health and safety are not taking into
consideration, workers may feel demotivated because of frequent accidents
occurring in the workplace and this may trigger unrest in the form of strike or
protest which may further lead loss in production time due to shut down of
operations.

Task 8: Human factors

Question 8
The organizational factors are the characteristics of the organization that affects the
way workers behave. Some of these negative factors that influence the health and
safety behaviour of workers are as follows:
1. Absence of safety procedure such as fire risk assessment. This will affect the
workers as they will not know the risk and hazard associated with the work place.
2. Poor health and safety culture of the organization: The health and safety culture
of the organization is negative as there is a blame culture as the directors
believed that every accident occur as a result of human error.
3. Lack of commitment and leadership from the management: The organization is
more interested in making money rather than given priorities to health and safety
in the workplace.
4. Lack of adequate resources: there are not enough personnel to support the HSM
in carrying out safety audit.
5. List of first aiders was not shared as seen in the scenario. This will lead workers
to reporting injuries to the wrong person.
6. Lack of consultation and workers involvement: Base on the scenario, audit and
risk assessment was done without involving workers. The workers may not

appreciate the findings and implementation of any policy thereafter may not be
taking seriously.
7. Lack of leadership interest in health and safety activity: The top management do
not have interest in health and safety as demonstrated in the scenario. Due to this
negative attitude, the HSM did show the finding of the audit to the director.
8. There was no leadership by example at MSL as the MD was asked me to falsify
document as demonstrated in the scenario where the MD wants me request that I
produce document demonstrating that management of health and safety at MSL
has nothing to do with the accident.
9. Approach to roles and responsibilities: There is no proper assignment of roles and
job description by top management as workers are seen carrying out the role of a
fire marshal even when they are not authorize to do it.
10. Lack of training: The workers may not have been trained properly on their health
and safety role as such find it difficult to report accident because from the
scenario the HSM doubted that all accident have been reported.
