1.0 General

This Health, Safety, and Environment (HSE) Bridging Document is established between Tanjung Offshore (Client) and
Intra Oil Services Sdn Bhd (IOSB), hereinafter referred to as the contractor, for the utilization of the accommodation
barge during the South Angsi Alpha Decommissioning Project. This document aims to delineate the responsibilities,
procedures, and HSE management systems applicable to internal barge activities and those associated with the
platform within a 500-meter radius.

1.1 Summary of Contract Scope of Work

Project Overview: The project involves the decommissioning of 13 wells located at the SA-A Platform in the
Terengganu High Seas. Tanjung Offshore Services Sdn Bhd (Client) and Perdana Services Sdn Bhd (Contractor) are
the primary stakeholders. The project will be assisted by a Barge and Anchor Handling Tugs responsible for towing
and anchor handling operations.

This document governs all HSE-related aspects associated with the accommodation barge operations during the
South Angsi Alpha Decommissioning Project. The scope includes tasks conducted internally on the barge and those
extending to the platform within the specified 500-meter Zone.

1.2 Purpose

The purpose of this document is to:

a. Establish clear guidelines for HSE responsibilities on the accommodation barge and the associated platform.
b. Ensure effective communication and collaboration between Tanjung Offshore and IOSB to mitigate HSE risks
during the South Angsi Alpha Decommissioning Project.
c. Provide a framework for the simultaneous application of distinct HSE Management Systems, based on the
nature of the work - internal barge activities versus platform-related tasks within the 500-meter Zone.

Bridging Elements:

HSE Responsibilities on the Accommodation Barge:


a. Development and implementation of the IOSB HSE Management System for internal barge activities.
b. Identification of key personnel responsible for HSE oversight on the accommodation barge.
c. Training programs to ensure all personnel understand and adhere to the internal barge HSE procedures.
d. Regular HSE audits and inspections specific to barge operations.
e. Emergency response procedures tailored for internal barge incidents.

HSE Responsibilities on the Platform within the 500-Meter Zone:


a. Utilization of Tanjung Offshore's HML HSE Management System for platform-related tasks.
b. Clear delineation of responsibilities between Tanjung Offshore and IOSB for platform-related HSE activities.
c. Joint HSE risk assessments and hazard identification for tasks that involve both the barge and the platform.

d. Coordination of emergency response procedures between Tanjung Offshore and IOSB in the 500-meter
Zone.

Communication and Collaboration:

a. Establishment of a dedicated communication channel between Tanjung Offshore and IOSB for real-time HSE
updates.
b. Regular joint HSE meetings to discuss ongoing activities, share lessons learned, and address any emerging
concerns.
c. Periodic review of the bridging document to incorporate lessons learned and continuous improvement
initiatives.

1.3 Methodology

The methodology used to develop this Interface Document is summarized as follows:


▪ Defined the scope of operations to be included;
▪ Reviewed the TOSSB and Perdana Nautika – HSE MS;
▪ Identified areas where joint effort is required by both parties to manage the risk;

1.4 Amendment

On occasions, such as the development of new procedures or significant revisions to existing procedures by either
party, it may be necessary to undertake a review of this document and make amendments accordingly. All adjustments
must be consistent with the HSE management of both parties and be sanctioned and agreed upon. TOSSB and
Perdana Nautika are accountable for the ownership of this Interface Document and therefore must take a joint role in
its maintenance. However, the ultimate custodian of the document is the TOSSB HSE Manager and his designated
HSE Lead through whom all potential changes are channeled.

3.2 Normal Communications

For the ongoing 'South Angsi Decommissioning' offshore project, routine operational communications will occur between the
Vessel Master and the platform, as well as with HML's ASB Marine Logistics Coordinator. Email will serve as the primary mode
of communication, supplemented by secondary channels such as VHF radio or telephone. Any additional communications
necessary beyond these established links will be determined as needed.

Prior to the vessel entering the 500-meter safety zone around the platform, the Vessel Master will ensure that the vessel's
standard 500M checklist is completed and signed accordingly.

All onshore communications between Tanjung Offshore Services Sdn Bhd (TOSSB) and Perdana Nautika will be initiated through
HML's Operations Department, Marine Logistics Division, HSE representative, and other relevant parties. Any further
communications required beyond this initial connection will be arranged as appropriate.

3.3 Interface Meetings


Daily Vessel Meeting

The Vessel Master will conduct a daily Toolbox Meeting on board to address operational and safety issues. Following
the morning Toolbox Talk, the Barge Master will also attend the daily operational morning meeting if required by
TOSSB. This ensures that marine crew activities do not interfere with ongoing operations at the platform. The Barge
Master will also present their Marine HSE I-See I-Act Card and Safety Performance, and raise any marine-related
matters requiring the attention of the TOSSB Operation Team and Project Management Team.

Internal Operational Meetings

The Vessel Master will conduct regular internal HSE meetings as outlined in their Health, Safety, Environment, and
Quality Management System (HSEQA -MS). These meetings will cover:

• Marine SOLAS Drills schedules as per established by Perdana Nautika


• Areas of concern raised during Toolbox meetings
• Planning for crew POB (Persons on Board) and crew changes
• Company audits and superintendent inspections
• HSE campaigns and field activities"

Weekly Safety Meeting

The Vessel Master, along with his officers, is required to attend the Weekly Safety Meeting as per the scheduled
sessions. These meetings serve as crucial forums for discussing and addressing various safety-related topics
pertinent to the vessel's operations. During these gatherings, participants engage in in-depth discussions regarding
safety protocols, hazard identification, risk mitigation strategies, incident reports, and lessons learned from previous
incidents or near misses.

Furthermore, the Weekly Safety Meetings provide an opportunity for the Vessel Master and officers to receive
updates on any changes in safety procedures, regulatory requirements, or company policies relevant to their roles

and responsibilities. It also allows them to share insights, experiences, and best practices to enhance safety
performance and promote a culture of continuous improvement.

Attendance at these meetings is essential to ensure that all personnel involved in the vessel's operations remain
informed, vigilant, and actively committed to maintaining a safe working environment. By fostering open
communication, collaboration, and accountability, these gatherings contribute to the overall effectiveness of the
vessel's safety management system and help safeguard the well-being of crew members, assets, and the
surrounding environment.

Safety Meeting Prior For Anchor Handling Operation

Prior to commencing an Anchor Handling Operation, it is imperative to conduct a comprehensive Safety Meeting,
which falls under the direct responsibility of the Barge Master. This meeting serves as a critical preparatory step to
ensure that all personnel involved in the operation are well-informed, adequately trained, and fully understand their
roles and responsibilities.

During the Safety Meeting, the Barge Master is required to convene all relevant personnel, including but not limited
to crew members directly and indirectly involved in the operation. This includes representatives from Tanjung
Offshore Services Sdn Bhd (TOSSB) and HML, as well as any other stakeholders deemed necessary. The purpose is
to ensure that everyone comprehensively understands the tasks at hand, potential hazards, and safety protocols
associated with the Anchor Handling Operation.

The Barge Master is tasked with facilitating open communication and collaboration during the meeting. This includes
calling upon the TOSSB Offshore Installation Manager (OIM) and HML representatives to provide insights, guidance,
and any pertinent feedback regarding the operation. All personnel are encouraged to actively participate, raise
concerns, and share any challenges they anticipate or encounter during the operation.

Moreover, the Safety Meeting serves as a platform for the Barge Master to accept advice or feedback from the
operation team, including TOSSB and HML representatives. This fosters a culture of mutual respect, accountability,
and continuous improvement. Any recommendations or suggestions provided by the operation team are carefully
considered and addressed accordingly to enhance safety performance and operational efficiency.

By ensuring that all personnel are fully briefed, prepared, and equipped with the necessary knowledge and
resources, the Safety Meeting plays a crucial role in mitigating risks, preventing incidents, and promoting a safe
working environment throughout the Anchor Handling Operation.

3.4 Reporting
Under the Reporting Section, the Barge Master is tasked with assigning either his Chief Officer or his Marine Safety
Officer to handle the reporting of various critical aspects concerning the vessel's operations. These reporting duties
are crucial for maintaining a comprehensive understanding of the vessel's status, ensuring compliance with safety
regulations, and addressing any issues promptly. Below are the specific reporting requirements:

a. Medical Health Status for All Crew:


This report provides an overview of the medical health status of all crew members on board the vessel. It includes
details such as any ongoing medical conditions, recent injuries or illnesses, and overall well-being assessments. This
information is essential for monitoring crew health and ensuring that appropriate medical assistance is provided if
needed.

b. Numbers of Reported Illness (if any) as Reported by Medic Onboard:


The report details any illnesses reported by the onboard medic. It includes the number of crew members affected,
the nature of their illnesses, and any medical interventions or treatments administered. This helps track health
trends among the crew and facilitates proactive measures to prevent the spread of illnesses on board.

c. Number of I-See I-Act Cards on Daily Basis:


This report documents the number of I-See I-Act Cards submitted by crew members on a daily basis. I-See I-Act Cards
are used for reporting observations, near misses, and safety concerns. Monitoring the submission rate of these cards
provides insights into the crew's awareness of safety issues and encourages proactive hazard reporting.

d. Life Saving Inspection on Weekly Basis:


This report outlines the results of weekly inspections of life-saving equipment on board the vessel. It includes checks
on lifeboats, life rafts, lifebuoys, and other essential life-saving appliances. Any deficiencies or maintenance
requirements identified during the inspection are documented, and corrective actions are taken promptly to ensure
the readiness of life-saving equipment in case of emergencies.

e. Fire Fighting Appliances on Weekly Basis:


This report summarizes the weekly inspection findings related to fire-fighting appliances on board the vessel. It
covers checks on fire extinguishers, fire hoses, fire detection systems, and other fire-fighting equipment. The report
highlights any issues detected during the inspection and verifies that all fire-fighting appliances are in proper working
condition to effectively respond to fire emergencies.

f. Planned Maintenance Schedule and Completed Tasks for the Barge/Vessel:


This report provides an overview of the planned maintenance schedule for the barge or vessel, including scheduled
maintenance tasks and their completion status. It ensures that routine maintenance activities are conducted as per
schedule to maintain the vessel's operational efficiency and compliance with regulatory requirements.

g. Marine Safety Officer Reports:


The Marine Safety Officer submits regular reports detailing safety-related observations, incident reports, risk

identify areas for improvement, and support the implementation of corrective actions to mitigate risks and enhance
safety culture..

3.5 Mobilization / Demobilization


The following procedures and specific Company offshore requirements shall be observed when mobilizing/
demobilizing from HML facilities:

Alongside any notices from Perdana Nautika, prominently displayed at a conspicuous location onboard, the TOSSB
HSE Policy, MAPO Guideline, and 'Life-Saving Rules' will also be prominently showcased, ensuring they are easily
accessible and available in appropriate languages for all personnel.
4.0 Responsibilities and Procedures

It is necessary to define roles, responsibilities, accountabilities and interrelations required to ensure that the HSEMSs
of both HML and contractor can be effectively implemented. Those that require input from both HML and contractor
are summarized in this Chapter.

It is noted that the roles and responsibilities outlined within this document do not impinge upon the underlying HSE
principle that any worker has the responsibility and right to not commence a task or interrupt any task assessed as
having an unacceptable level of risk.

4.1 Responsibilities

Proper and effective communication between the TOSSB management, Perdana Nautika Onsite Marine
Representative and the contractor management is of paramount importance. Individual responsibilities and
accountabilities of personnel engaged in these operations are clearly defined (Tables 1 & 2) and understood.
Delegated responsibility / accountabilities below the levels described are detailed in the respective Management
Systems of TOSSB and contractor.

Each party is responsible for its own contractors (and sub-contractors). Proper and effective communication
between the TOSSB Representative and contractor HSSE Manager is of paramount importance.

5.2 HML Emergency Management Organization


The HML Emergency Management Organization, facilitated by the Incident Management Team (IMT) and Business Support
Team (BST), will provide assistance to the offshore Tactical Response Teams (TRT). In the event of an offshore emergency

5.3 HML-TOSSB Emergency Management Organization


The TOSSB-HML emergency management organization, illustrated in the figure below, outlines the procedures for responding
to emergencies at the vessel facility. In the event of an emergency, the site emergency response team will be promptly activated, with
escalation conducted as depicted in the flowchart. The Platform OIM will notify the respective HML CSR of the emergency,who
will then inform the SA-A OIM and Incident Commander based on the severity of the situation. If required, the HML IMT Incident

5.4 Medical Evacuation


The HML Stand-by Contract with the Helicopter Provider encompasses all personnel stationed on HML’s offshore platform or
HML-chartered vessel, irrespective of their employer. In the event of an emergency, the HML SA-A Offshore Installation
Manager (OIM), also known as the On-Scene Commander, will be responsible for determining and initiating any necessary
evacuation (Medevac / Casevac / Bodyvac) of an injured, sick, or deceased individual aboard the vessel. This decision will be
made after consulting the Barge Medic or vessel Master for guidance on the urgency and nature of the evacuation.
The medical evacuation process for HML is outlined in Figure 6. The nearest helideck, located at Perdana Excelsior (Barge), will
be utilized. The Vessel Master will coordinate with the TOSSB OIM to ensure the safe transfer of the Injured Person (IP).
Subsequently, the Medevac helicopter will transport the IP from the Barge helideck to the Kota Bahru Helibase.
Upon activation of the medical evacuation, the HML Incident Commander on duty will liaise with the TOSSB OIM and contractor
Onshore Emergency Response Team (ERT) to organize transportation and standby personnel at the Kota Bahru Helibase.
Once the IP reaches the shore, it becomes the contractor's responsibility to arrange medical care for all their employees and
subcontractors. In the event of serious injury or fatality, the Malaysia Police Department and ambulance service will be
mobilized at the Kota Bahru Helibase, if necessary.

If the IP is an employee or subcontractor of the contractor, it is the contractor's duty to notify their next of kin. The HML
Incident Commander on duty will inform and furnish relevant information to the respective contractor ERT or Operation
Manager to facilitate next of kin notification.

5.5 Security / Vessel Interference


5.5.1 Ship Security Plan
When applicable, the marine vessel must adhere to the guidelines outlined in the International Ship and Port Facility Code and
possess a Ships Security Plan that has been approved by the Flag state of the vessel. The security level stipulated in the Ships
Security Plan is determined by the Flag State/Contracting Government. The Master or the Ship Security Officer (SSO) bears the
responsibility of declaring and implementing the ship security level. These security levels, or readiness conditions, establish
protocols to address security threats or breaches, ensuring the continuity of critical vessel operations or vessel/port interface
activities. Under normal operating conditions, Security Level 1 is maintained.
All personnel will receive clear guidance on security levels through announcements made via the PA system and notices placed
around the vessel. Training sessions will be conducted at all readiness conditions to ensure swift responses to evolving security
challenges. Upon entering a port, the vessel must adhere to the security level mandated by the Contracting Government. The
SSO has the discretion to operate at a heightened security level, irrespective of recommendations by the Port Facility Security
Officer (PFSO).
Close coordination between the SSO and Port Facility Security Officer is imperative, with both parties required to liaise and
synchronize actions accordingly. If the vessel is operating at a higher security level than the port, the SSO must promptly inform
the PFSO and the competent authority of the Contracting Government overseeing the port facility's jurisdiction. At Security
Levels 2 and 3, the SSO must acknowledge the designated authority's advice regarding changes in the security level and confirm
the vessel's security status to the Port Facility Security Officer, reporting any discrepancies in implementation.
During standby operations at the SA-A field, the Officer On Watch (OOW) aboard the vessel must remain vigilant, monitoring
for any unauthorized marine vessel incursions or the presence of Foreign Law Enforcement Agency Assets (FLEAA), Navy, Coast
Guard vessels, or commercial/fishing vessels within the SA-A field. If any unauthorized vessels or FLEAAs are detected, the Vessel
Master must promptly report the incident to the platform radio room and SA-A Marine logistics for further action. Following
instructions from the platform OIM, the Vessel Master shall attempt to establish communication with the unauthorized vessel
to ascertain its identity and instruct it to vacate the field.

5.6 Extreme Weather


Perdana Nautika's Emergency Pull-Out Procedure, the HML MAPO matrix, is to be consulted when conducting operations at SA-
A offshore facilities. This procedure specifically addresses activities and operations that may be impacted by adverse weather
conditions. These activities include:

• Working over the side


• Helicopter operations
• Crane operations
• Ship-to-ship transfers
• Standby/supply operations
• Well operations
• Other routine activities susceptible to adverse weather conditions
• AWB Operation

The vessel Master is tasked with taking necessary measures to avoid navigating through severe weather conditions. The Master
assumes full responsibility for all decisions made and must assess the need to evacuate non-essential crew and personnel from
the vessel

It is incumbent upon the Master to review the weather forecast provided by HML and strategize vessel operations and voyages
accordingly. Prior to the onset of severe weather, the Master must instruct all onboard crew to prepare the vessel and mitigate
potential accidents or injuries that may occur due to adverse weather conditions.

5.7 Media Relations


An integrated strategy will be adopted for any media responses beyond the local level. This approach will be determined jointly
by the Business Support Team (BST) for HML and the TOSSB Team, as per standard procedural protocols. This decision-making
process will be integrated into the Incident Management Systems and Emergency Response frameworks of both TOSSB and
HML.

Under no circumstances should the Contractor, vessel operator, or their subcontractors conduct a press conference or release
any media information without prior approval from TOSSB, HML, and MPM (PETRONAS).
