In the framework of World Oral Health Day, FKG Unbrah


students held a scientific seminar by inviting many
speakers in the field of dentistry. One of the speakers, drg
X, is an Oral Specialist, delivered material on the effect of
stress on oral health. He said that the metabolic processes
of nutrition, behavior, reproduction, growth,
gastrointestinal, cardiovascular, nervous and renal systems
and their various responses to all forms of stress, all of
these functions are coordinated by the endocrine system.
The emergence of stress will activate the hypothalamic-
pituitary-adrenal axis (HPA axis). The activation of the
HPA axis will stimulate the adrenal glands to increase the
secretion of the hormone cortisol through a negative
feedback process. One of the clinical features in the oral
cavity due to stress is the appearance of Wickham striae on
the oral mucosa.

The endocrine system is a control system of ductless glands that produce


hormones that circulate in the body through the bloodstream to affect other
organs. The endocrine system is closely related to the nervous system,
controlling and integrating body functions. Endocrine glands will release their
secretions directly into the blood. The endocrine glands consist of the islets of
Langerhans and parathyroid, gonads, adrenal glands, pituitary, thyroid and
thymus. Endocrine glands together with the nervous system control, and these
two systems work together for body homeostasis, and secrete chemical
compounds in the form of hormones and distribute them through the blood to
cells/tissues/organs towards the target.

1. pituitary gland (pituitary)


The pituitary gland is a cell located at the base of the brain and is connected to the
hypothalamus by the pituitary stalk, or hypothalamic infundibulum.
2. Sex Glands (gonads)
The gonads are the main reproductive (sex) organs, consisting of the testes in males and ovaries
in females. Apart from being an exocrine gland to produce sperm, the testes also act as an
endocrine gland in secreting the hormone testosterone (androgens).
3. Thyroid gland.
Located at the front of the neck is very important in metabolism, this thyroid gland is in the form
of twin cups and between them is an area that is arranged in layers like a roof. The thyroid gland
detects two types of hormones, namely: Thyroxine or tetraiodothyronine (T4), 90% of the total
secretion of the thyroid gland and Triiodothyronine (T3), secreted in small amounts.
4. Parathyroid glands
The parathyroid glands are four small organs each about the size of an apple seed, located on
the anterior surface of the thyroid gland and separated from the thyroid gland by a connective
tissue capsule. Produces parathyroid hormone which plays an important role in phosphate
metabolism and calcium metabolism.

There are several types of hormones, including steroids, amino acid


products, as well as polypeptides and proteins. The hormones differ
in their molecular structure (eg, size and chemical properties). Due to
differences in structure, their mechanisms of action (eg, whether
they can enter target cells and how they modulate the activity of
these cells) are also different. Steroids, which are produced by the
gonads and parts of the adrenal glands (eg, adrenal cortex), have a
molecular structure similar to that of cholesterol. The molecules can
enter their target cells and interact with receptors in the fluid that
fills the cell (i.e., the cytoplasm) or in the cell nucleus. The hormone
receptor complex then binds to certain regions of the cell's genetic
material (i.e. DNA), thereby regulating the activity of certain
hormone-related genes.

1. Cortisol hormone
Cortisol hormone is produced by the kidneys, which produces glucocorticoids,
causing a decrease in the function of SIga, Ige and neutrophils, causing
decreased immune function against bacteria. If there is excess cortisol, bacteria
can thrive, and cortisol is the main stress hormone, RAS is a manifestation of oral
disease.

2. Adrenaline hormone
Adrenaline is a hormone produced by the adrenal glands after getting a signal
from the brain when a stressful situation arises. Adrenaline cooperates with
another stress hormone, namely norepinephrine which is responsible for deciding
the reaction when stress arises.

3. Norepinephrine
This hormone is the same as Adrenaline which is secreted by the adrenal glands
and comes from the brain. The function of the hormone Norephnephryne is to
keep us focused and awake during stress, we will become more alert, unable to
sleep, and focus on problems.
