During routine inspection it was noticed by Mr. Dinesh


Kumar(3/O) that the provision crane lashing wire (one strand)
had come out and the wire was replaced immediately and
provision crane was secured properly

Prior arrival Boryeong all the mooring ropes which where on the
drums were uncoiled and the same
properly coiled on the drums to avoid the ropes getting struck in
the drums during arrival mooring operations

During STS Operations observed the additional mooring ropes


were secured on the bollards instead of keeping the ropes on
warping drum. Crew were encouraged to follow safe mooring
practices at all times

CE - When making entry into enclosed spaces a gant line to be


attached to harness & a compressed air line is attached , in
case of emergency the person may be assisted to come up with
gant line & compressed air to clear the immediate atmosphere
