EVALUATION FORM

Primer on “ How to conduct an Earthquake drill in school?”

Dear Sir/ Madam:

We would like you to evaluate the effectiveness of the PHIVOLCS primer on “How to conduct an
Earthquake Drill in school” in order to improve it further.

1. Are the instructions in the primer very clear? If not why?

The instructions are clear.

2. Which of the primer is difficult to implement and why?

The instruction which says: “Keep calm and don’t panic” is difficult to implement because students
will not calm down so easily.

3. In order to improve future drills, which part of this primer needs improvement?
In the dissemination of the primer, It would be better if we include the SSG and CAT Officers for their
assistance during the drill as part of their duty in the school. All teachers also will integrate it to their
subjects.

4. What do you think would be the form of media that this primer be best represented? Example: Poster, VCD,
Comics, etc.
I think the presentation of the primer should be attractive and presentable. We could have it in the
form of poster, comics, or power point presentation.

FIRST QUARTER EARTHQUAKE DRILL REPORT S.Y. 2014-2015

I- SCHOOL DETAILS

Date/Time: July 11, 2014 @ 9:00 am

Place of Drill: SAN JOSE NATIONAL HIGH SCHOOL

Location during the drill: San Jose, Bislig City

II- DATE OF EARTHQUAKE DRILL July 11, 2014

III- Written Scenario

Before the start of the drill, we conducted an earthquake drill orientation. All students and teachers
gathered in the library as the place for the orientation. The speaker discussed to the students the proper
procedure before, during, and after the earthquake as their preparation for the drill right after the orientation.
We presented video presentations about Earthquake Preparedness for their additional knowledge. After the
short orientation, we let the students go back to their respective classroom to continue their classes. For
about 30 minutes we assigned student officers to assist for the unannounced drill. They formulated different
teams: the information group, the medics group and rescue group. The drill went smoothly and no problems
encountered.

IV- School Evacuation Plan

The site for evacuation is in the open ground of the school campus where there are no trees, buildings,
and things that can harm the students. It is spacious and free from distraction.

V- Activities Conducted Prior to the Conduct of Earthquake Drill

Earthquake drill will be conducted quarterly. Teachers integrate it in their classes.

VII- Government Agencies Involved:

VIII- Problems Encountered:

None

IX- How does the school address the problems?

Not applicable because there was no problem encountered.

X- Recommendation:

a. It would be better if memos on SIMULTANEOUS EARTHQUAKE DRILL should be given earlier as well as its
comic primer.

b. In the dissemination of the primer, I think it would be better if all teachers will be given a copy so that it
would be integrate in each subject properly.

c. It would be better too if there would be a meeting of all school heads and school coordinators on
earthquake drill activity together with the designated division coordinator on the conduct of the drill so that
there would be sharing of ideas, experience and suggestions that would enrich the school based activity.
