1 PURPOSE
PETRONAS intends to build a grass root integrated Refinery and Petrochemicals Complex by
developing the RAPID PROJECT (Refinery and Petrochemical Integrated Development) to
meet both domestic and Asia's energy and chemicals demand. The RAPID PROJECT consists
of a world scale integrated site which includes Refinery with a deep conversion scheme, 300
000 barrels per stream day capacity, Naphtha Steam Cracker of 1.28 million tons of ethylene
per year capacity; Petrochemical Derivative Units, utilities, off site and jetty installations.
The RAPID PROJECT is a fundamental component of the Pengerang Integrated Complex
(PIC) which also includes the following major projects; Pengerang Cogeneration Plant (PCP), a
new LNG Regasification Terminal (RGT2), Raw Water Supply Project (PAMER), Pengerang
Deep Water Terminal (PDWT) and Air Separation Unit (ASU) which provides oxygen and
nitrogen gases to the Complex and to the local market.
The RAPID PROJECT is located at Pengerang, Mukim of Pengerang, Kota Tinggi District,
Johor and is in close proximity to the international shipping lane connecting the Straits of
Malacca, Singapore and the South China Sea.
The purpose of this document is to provide the procedure of piping works include pipe supports
for Unit 6900 which conducted prior to, during, after completion of shop pre-fabrication and field
erection of piping works.
This method of statement covers for Confined space entry activities at RAPID Site.

3 ACRONYM AND DEFINITIONS


OWNER: PRPC Utilities and Facilities Sdn. Bhd.
EPCM CONTRACTOR: OUI JV: Joint venture appointed by OWNER with specified
authority to perform the EPCM Services of the UIO Project.
PROJECT or UIO PROJECT: EPCM of utilities, interconnecting and offsite facilities (UIO)
Package 14 for RAPID PROJECT at Pengerang, Johor
EPCM: Engineering, Procurement and Construction Management
PACKAGE WORK: The works, services or supplies, or any or all of the foregoing,
to be undertaken by a PACKAGE CONTRACTOR or
PACKAGE VENDOR, as applicable.
"SCOPE OF WORK" may also be used instead of "PACKAGE
WORK" to facilitate the understanding of all the parties.
PACKAGE: mmeans each package of works or supplies forming part or
parts of the PROJECT.
PACKAGE VENDOR: means any Vendor engaged by OWNER to supply Equipment
and Materials or Spare Parts, which may include associated
services.
PACKAGE CONTRACTOR: means any Contractor engaged by OWNER to undertake the
PACKAGE WORKS for a PACKAGE.
PROJECT SPECIFICATION means mandatory documents comprising of specifications,
PETRONAS Technical Standards and Project Procedures.
SUBCONTRACTOR: means any company which EPCM CONTRACTOR has
engaged or proposes to engage (as the context may require) to
undertake any part of the EPCM CONTRACTOR Services
Greenfield Site New construction site or area being developed with no existing
above or underground operational services (e.g. chemical or
hydrocarbon pipelines, storage tanks, electrical cables and
power lines) which pose significant HSE risks to the project
during execution of construction.

4 RESPONSIBILITY

4.1 CONSTRUCTION MANAGER


 Report to Project Manager.
 Overall site management of construction activities.
 Ensure construction works undertaken in accordance with quality specification, safety
and security requirement.

4.2 CONSTRUCTION ENGINEER


 Report to Construction Manager.
 Coordination works and overall supervision between Construction, QA/QC, Material
Coordination and Safety.
 Generate documentation for construction works.

4.3 SUPERINTENDENT
 Report to Construction Manager and Construction Engineer.
 Overall site representative for construction works.
 Overall site supervision and coordination.

4.4 HSE MANAGER/SITE SAFETY SUPERVISOR


 Review the Safe Work Plan and Work Permit prior to work execution.
 Verify all equipment’s and power tools are in a good and safe working condition.
 Ensure safety requirements are implemented and adequate safety measures are
undertaken.
 Monitor the safety of overall work execution.
 Verify personnel involving construction works are undergone safety induction, training
etc.

4.5 AUTHORIZED GAS TESTER (AGT)


 To conduct gas testing prior to hot work, confined space entry or other types of work
where gas testing of work where gas testing may be required.
 To identify all the chemical hazard and recommends control measures to ensure safe
entry.

4.6 ENTRY SUPERVISOR


 Supervise all the activities before, during and entry operation.
 To check and verify the appropriate entries (testing, procedures and equipment) that
have been made on the Permit to Work (PTW).
 Be in charge on communication during the emergency.

4.7 AUTHORIZED ENTRANT


 To perform work assigned inside the confined space.

4.8 STANDBY PERSON


 To continuously maintain an accurate count of authorized entrants in confined space
and to ensure that the means used to identify authorized entrants accurately identifies
who is in the confined space.
 To monitor activities outside and inside of every confined space to determine if it is safe
for entrants to remain in the confined space and orders to the authorized entrants to
evacuate a confined space immediately if there is an emergency.
 To make sure the number of authorized entrant is tally to the PTW applied.

5 DEFINITION

A confined space is an enclosed or partially enclosed space that:

 has restricted means of entry or exit


 is not primarily designed or intended for human occupancy.
 large enough to work so the worker can enter and perform the task
 is at atmospheric pressure during occupancy
 the excavation activities more than 1.5m deep

Besides, the confined space also can represent a risk for the for the health and safety of anyone
who enters, due to one or more of the following factors:

 its design, construction, location or atmosphere


 the materials or substances in it
 work activities being carried out in it, or the
 mechanical, process and safety hazards present

Example of confined space:

 Enclosures with limited openings; pipe with large diameter


 Storage tanks and pressure vessels
 Silos
 Space located below ground or deck level; sump pit, drain pits, associated tunnels

6 HAZARD IN CONFINED SPACE

Confined space can be a deadly space if necessary actions is not taken properly. Hazard that
are present during the confined space activities:

 Poor air quality: There may be an insufficient amount of oxygen for the worker to breathe.
The atmosphere might contain a poisonous substance that could make the worker ill or
even cause the worker to lose consciousness. Natural ventilation alone will often not be
sufficient to maintain breathable quality air.
 Chemical exposures due to skin contact or ingestion as well as inhalation of 'bad' air.
 Fire Hazard: There may be an explosive/flammable atmosphere due to flammable liquids
and gases and combustible dusts which if ignited would lead to fire or explosion.
 Process-related hazards such as residual chemicals, release of contents of a supply line.
 Noise.
 Safety hazards such as moving parts of equipment, structural hazards, entanglement,
slips, falls.
 Radiation.
 Temperature extremes including atmospheric and surface.
 Shifting or collapse of bulk material.
 Barrier failure resulting in a flood or release of free-flowing solid.
 Uncontrolled energy including electrical shock.
 Visibility.
 Biological hazards

7 COMMUNICATION

An adequate communication system is required to enable:

 Communication between people inside and people outside the confined space.
 Help to be summoned in an emergency.
 Emergency rescue procedures to be initiated.

Systems can include speech, tugs on a rope, walkie-talkie etc. Equipment to be used in
potentially flammable or explosive atmospheres should be specially protected so they do not
present a source of ignition.

8 PETRONAS ZERO TOLERANCE RULES

PETRONAS have establish Zero Tolerance (ZeTo) principle to ensure all activities are carried
out in a safe manner with non-tolerance of non-compliance to these ZeTo rules.

The ZeTo rules include:

 Work with valid PTW required for the job.


 Verify energy isolation before starting work.
 Obtain authorization before overriding or disabling safety critical equipment.
 Obtain authorization before entering a confined space.
 Protect yourself against a fall when working at height.
 Use the correct personal protective equipment (PPE) when handling hazardous chemical.
 Obtain authorization before excavation or entering a trench.
 Do not position yourself under a suspended load.
 Do not smoke outside designated area or bring potential ignition sources into the process
areas without authorization.
 Do not use your mobile phone/walkie-talkie while driving, follow the speed limit and use
your seat belt.

Thus, the ZeTo rules applicable to this method of statement is:

9 WORKING INSIDE THE CONFINED SPACE

9.1 PERMIT APPLICATION

At RAPID Site, a confined Space Permit, Form RAPID-P0014-OUI-HSE-PRC-0001-F3181, must


be issued for employees before proceed to enter the confined space. No person shall enter or
work in a confined space without a valid entry permit.

 This form is referred to in the RAPID-P0014-OUI-HSE-PRC-0001-3109 Greenfields Permit


to Work procedure.
 The appointment and training of Confined Space Entry Permit Authority in referenced
RAPID-P0014-OUI-HSE-PRC-0001-3109 Greenfields Permit to Work
 Permit Authority’s and permit Requesters required for approving or completing a Confined
Space permit and need to follow Malaysian DOSH requirements for the Confined Space
entry training.
 The package Contactor and Package Vendor Permit Requesters shall complete the
RAPID-P0014-OUI-HSE-PRC-0001-F3196 Confined Space Entry Hazard Identification
Guidelines checklist to accompany the Job Method of Statements (JMS), Job Safety
Analysis (JSA) and Permit to Work (PTW).
 All the required confined space permit documentation shall be available at the confined
space entry point.
 At RAPID Site, a confined Space Permit, Form RAPID-P0014-OUI-HSE-PRC-0001-F3181,
must be issued for employees before proceed to enter the confined space.
 This form is referred to in the RAPID-P0014-OUI-HSE-PRC-0001-3109 Greenfields Permit
to Work procedure.

9.2 RISK ASSESSMENT

The risk assessment may highlight a need to check that the atmosphere is free from both toxic
and flammable vapors and that there is an adequate concentration of oxygen prior to entry. A
competent person using a suitable gas detector, which is correctly calibrated, must carry out
testing. Where the risk assessment indicates that conditions may change, or as a further
precaution, continuous monitoring of the air may be necessary. Test results must be recorded on
the permit to enter.

The test equipment used in confined space will include the following measurements:
 Oxygen Level
 Lower Explosive Limit (LEL)
 Toxic substances i.e., H2S, CO (Depend on confined space environment)

The person may not to enter the confined space unless the testing conducted shown:

 The oxygen concentration is in range 19.5% to 22.5%.


 The atmosphere is not explosive, with flammable levels less than 10% of LEL.

The oxygen content must be between 19.5% to 22.5% since if the oxygen content less or more
than these percentages will often cause “false readings “for flammable levels.

 The concentration of airborne toxic materials is below the action level (less than ¼) of the
occupational exposure limits (25% of OEL). The external atmosphere is safe from toxic
materials.
 A differential between outside and inside unsuspected contaminations that may be
present.
 The temperature in the confined space is not or will not become hazardous.
 Adequate ventilation will prevent the accumulation of hazardous atmosphere.

In event of a monitoring alarm or other emergency stopping the work being carried out, the
recipients will discuss the conditions with the Confined Space Entry Authorized Permit Issuer
before recommencing work.

9.3 HOT WORK AND INSPECTION INSIDE LARGE DIAMETER PIPE

Internal pipe may require welding, cutting, grinding and other hot work. Since it has a restricted
means of entry and exit, it become a “confined space”.

9.3.1 Actions Required Before Carry Out Hot Work in a Confined Space

1. Ensure the PTW has applied and all equipment has been checked.
2. Eliminate any condition that would make removal of the confined space entry cover unsafe.
3. Open all covers and secure them from closing.
4. Test the space for oxygen content, combustibles and any other potential toxic gases to
assure justification conditions is met.
5. Provide means for readily turning off power, gas and other supplies from outside the confined
space.
6. Protect or remove any hazardous materials which may become hazardous when exposed to
hot work.

9.3.2 Actions Required during Hot Work in a Confined Space

1. Provide continuous ventilation and monitor air condition.


2. One attendant is required outside the permit space for the duration of entry operations.
3. Ensure the is no equipment or materials is blocking the exit or rescue effort.
4. To maintain continuous communication between personnel inside and outside the confined
space.
5. Lockout/tagout procedures will be followed on all applicable equipment

9.3.3 Safety Pre-cautions

Even when the confined space is tested and certified safe for any person to enter, at least one
person in a group working in the same vicinity shall be equipped with suitable instrument for
measuring oxygen, combustible and the identified toxic contaminants

In addition to the continuous ventilation requirement, it is essential to conduct the periodic


retesting, taking the following factors into consideration

 The possibility or likelihood of a change in the space by the potential release of the
hazardous materials.
 When a confined space is vacated for a significant period of time ≥ 30 min, without the
space being monitored continuously.
 When the atmospheric hazards in a confined space is detected by the confined space
safety assessor during periodic testing or continuous monitoring, all persons in the
confined space shall vacate the confined space immediately.
 An evaluation shall be made to determine how the hazardous atmosphere was developed.
 No person shall re-enter the confined space until it has been re-certified safe for entry and
a new confined space entry permit is issued.

9.4 PERSONAL PROTECTIVE EQUIPMENT (PPE)

All the PPE requirements shall be specified on the Confined Space Entry permit. Details of
PPE requirements will be listed in RAPID-P0014-OUI-HSE-PRC-0001-F3196 Confined Space

 Breathing apparatus (for poisonous area or oxygen deficiency)

 Air Blower – To supply fresh air for personnel inside the confined space
 Signage – To avoid unauthorized person and vehicle from entering the working area.
 Communication Device – To maintain communication between personnel inside and
outside the confined space
 Body Harness – Equip the rescue team for entering the confined space
 First Aid Box – For first aid treatment in case emergency happen.

9.5 CLOSURE OF A CONFINED SPACE

On completion of work, the of Authorized Permit Authority to acknowledge in writing that the
work has been completed and all persons involved in the work have left the confined space
and have been accounted for.

Is a duty of Authorized Permit Authority to check the confined space that no person remains
inside and the equipment & materials has been removed from the work area.
