1. Reviewed Method Statements, Material Submission Approvals, and Inspection and Test Plans

3. Karak Bridge No. 17 – Site clearing and preparation works are ongoing
4. Bentong Tunnel No. 3 – Tunneling works continues with installation of 18 th & 19th steel rib (until
CH244.34) and shotcreting. Works recommenced today
5. Bentong Tunnel No. 2 – Intercepting ditch formwork installation continues. Site Clearing and
removal of Earth into Majlis Bentong Storage Yard area.
6. Kg. Temiang Tunnel No. 1 – 7th to 8 th stage slope protection works. Excavation and installed 43
nos of T20 anchor mortar bolts, installing BRC mesh A8 and shotcreting G30, 150mm thickness for
slope protection. 4th stage of intercepting ditch formwork installation and concreting 22.0m of side wall.
7. Genting Tunnel – Foundation slab centre portion rebars fixing in progress till 2m height. Site
clearing and construction of workers quarters and site offices.
8. Karak Crusher Run Bridge No. 3 – Site clearing works, construction of site offices and workers
quarters in progress. Awaiting the official report for the 1 No. of Statnamic Load Test and 1 No. of Bi-
Directional Test. Continuing with the installation works for the Ready Mixed Concrete Batching Plant
No. 1
9. Section 7 Laboratory Centre, Testing and Sampling - Concrete cubes compressive strength tests

CCC has obtained TNB verbal approval to carrying out works in TNB protective zone.
Subsequently, we have also noticed that the aggregate storage yard at the Ready Mixed Concrete
Batching Plant No. 2 is being built underneath the TNB rentice zone and that has been highlighted
to CCC staff

In addition, at Bentong Tunnel #3, the installations for the 42mm dia. advance small pipes at 10 – 15
degree inclination, drilling of holes in the portal frame web steel ribs are awaiting designer
confirmation in English. In addition to, CCC staffs have yet to make available the gauge to measure
the welding thickness, the welders’ certificates and welding tests for the steel ribs. Meanwhile,
installation of steel rib frames at Bentong Tunnel #3 have recommenced today. Subsequently, CCC
has yet to provide the Geological Mapping report in English to the SC. The Railway Tunnel
monitoring measurement code Q/CR 9218-2015 and Q/CR 9247-2016 as indicated in the drawings
would also need to be translated into English. We have also highlighted on possible typo error on
indicated BS EN 1101 instead of BS EN 1011 code for welding works being stated in the drawings,
which the CCC Tech. dept. is checking on the matter

Genting Tunnel instrumentation and monitoring initial readings were not provided to the SC. The
ground bearing capacity at the portal end wall and excavated replacement section records have
been provided to the SC but some portions are in Chinese and needs to be translated into English.
SC has also detected the temporary drainage constructed beside the portal and excavated
replacement sections to channel the water into the silt traps and river. No completed
documentations provided to the SC. Casting of RC structure with thickness above 500mm is defined
as mass concreting as stipulate in specification clause 18.9.1, and thermal and shrinkage effects on
the structure need to be addressed.

Karak Bridge #3, the Statnamic Load and Bi directional test official report is still pending from CCC.

Bridge no #17, no RFI was raised for site clearing activity on site.

SC is still awaiting monthly materials summary reports and analysis from CCC Laboratory Dept.
CCC staffs were not familiar and aware of the requirement in the specifications during testing of the
materials

SC has requested on numerous occasions previously both verbally and through emails, discussions
and meetings the above-mentioned items to be provided. Please provide the above-mentioned
documentations as requested to the SC the soonest

Furthermore, SC have also noticed and informed CCC staffs that the ESCP were not completely
implemented at worksites, and could possibly affect the quality of water being discharged into the
rivers and public domains, while the dirty vehicles leaving the worksites were not cleaned as the
wash troughs were not utilized, thus leaving muds and earth onto the public roads at the Genting
Tunnel entrances. As highlighted by CCC HSE Dept. during the monthly CCC/MRL/SC Safety
Committee meeting No. 3 on 28/5/18, there are still many workers/staffs that do not possess the

CIDB cards and attended the Confined Spaces course, while the welders do not possess the proper
certifications as required in Malaysia. CCC to expedite to resolve these matters urgently.

4.1 General Introduction
The project was located at Bentong and was registered with DOSH Pahang PH/17/03/114734.
The alignment of the project ECRL as approved in the schedule 2A EIA (REF:JAS
50/013/602/007(31) dated 20th June 2017. We have highlighted numerous safety non compliances
to CCC staffs like welding without proper PPEs and usage of non approved S/S/Os to name a few

Main Problems:

Problem

 To date, SC only received some of the IFC drawings and Technical Specifications that were affecting the
site inspection works and reviews of MS and MSA documentations.
 Sections were unfamiliar with the local authority requirement required procedures before works
commencement.
 Kg Temiang Tunnel No. 1 boundary survey on government and private land to be confirmed.
 No RFI was raised for the site clearing activity at bridge #17.
 CCC staffs and workers, and SC staffs did not attend the necessary trainings on QAQC, HSE, DC, and
other required trainings/courses

Solution

 To provide the respective IFC drawings and Technical Specifications the soonest so as to not affect the
documents’ reviews and work inspections.
 Sections to familiarize with the Authority bylaws and get the relevant approval from the relevant Authority.
 Kg. Temiang Tunnel No. 1 boundary survey on government and private land to be carried out and
confirmed.
 Section to raise RFI for every activity accordingly.
 To carry out and attend the necessary trainings/courses on QAQC, HSE, DC, and other required trainings

5. Lanchang Precast Yard.

 Continue base preparation works for T-beam casting yard at Lanchang


 Construction of site offices and workers quarters ongoing
 Ongoing installation works for ready mixed concrete batching plant no. 1.
 Ongoing site clearing works

The following activities would be performed by the SC:

 Reviewing of document submissions from Sections


 Attending to RFIs for works on site
 Witnessing of material samplings and testing as requested by Sections
 Closure of RFIs submitted
 Continue site patrols and monitoring of works on site with respect to HSE
 Close and continuous liaison with CCCC HQ and Sections on issues raised and to be resolved
