Verification of Previous Meeting Minutes

No Previous minutes of meeting due to just held on 23thNovember 2017 was reviewed by

Other EHS Matters

AZSB
3.1  AZSB needed to give second induction to their new workers due to PPE Info
violation
 Its included safety shoes compliments, only wellington boot will be approve for
wet work
 Penalty will be given to workers that breaking the rules
4.0  Supervisor or 3S needed to apply permit to work (PTW) before start the works,
and PTW apply must related to the works, any addition of works can be request
4.1 when apply PTW.

 AZSB can provided temporary tool box for their workers to store PPE for
entering bonded area.
 Workers are given a leeway for not wearing PPE at 8.00-8.30am before start
the works
 Working at high, the workers who did not wearing safety harness are already
given the safety stand down and AZSB need to take serious this action to avoid
it happen again in future.
 Safe working AZSB are increase from 2017 to 2018
 AZSB should appoint a safety representative among current employees to
monitor their employees when safety official are absent or not on site.

 Dragging crew must parked their vehicle at authorized placed only, and must Info
follow TBSB rules to reverse parking.
 Planning to enter the dragging boat for walkabout and inspections.

 Suggested to build walkie talkie tower, for critical works such as lifting that
needed strict supervision.

 GENERAL ISSUE
 Motorists in the TBSB area need to wear motorist’s helmets, preferably a hat
that can protect their ears. Each vehicle that entering TBSB area must be
complete the document by law, e.g. road tax, insurance body to smooth the Info
process if any accidents happen.
 Compliance with PPE for VIPs and guests is not mandatory unless they are
working, PPE is mandatory
 Car park area in TBSB is a free zone PPE
 In the event of an accident, 3 things to be asked by dosh are
1. Does the company tell you about the hazard at work?
2. Does the employer provide training?
3. Has risk assessment been made?
 Safety helmet issue, each employee must follow the color coding that the
company has set up to avoid confusion
 Procedural penalty has been made, will be implemented later this month
 Application of PTW must be in accordance with the work requirements,
supervisors need to manage PTW for their employee
 For fogging issues, request fogging machines with AZHQ, if the application is
rejected, the TBSB will find a third party or KKM to resolve this issue.
 Next month planning, going to KKM for HIV AIDS training
 Smoking is permitted only in the designated area, the description has also
been provided during the induction
 For machinery inspection, color coding has been set for which 1-6 months of
the sticker is green, 7-12 months yellow and red to reject which machinery
cannot work properly

As there were no further matters arising, this meeting was adjourned at 1200 hours.
