MEMORANDUM OF AGREEMENT

between FAMILY CERNECHEZ &

BASUD ELEMENTARY SCHOOL on the
TEMPORARY USE OF ONE ROOM AS READING STATION CENTER

KNOW ALL MEN BY THESE PRESENTS:

This memorandum of Agreement is entered into by and between;

THE BASUD ELEMENTARY SCHOOL, represented by Mrs. Josephine B. Magas;

AND

THE FAMILY CERNECHEZ represented by Janet B. Cernechez;

WITNESS

Article I

COVERAGE

This agreement cover the use of ONE ROOM as temporary reading center in support to the
Department Education Program “BRIGADA PAGBASA 2021” of Basud Elementary School.

Article II

SCOPE AND IMPLEMENTATION

1. This agreement specifies the use of one apartment unit located at Purok 4 Cobo,
Tabaco City owned by Family Cernechez.

2. The family offered the said apartment unit to support the pupils of the school who were
classified as Struggling Readers and Non-Readers.

3. The school will conduct an inspection as to what improvement and sort of renovation
should be done to have conducive learning station.

Article III

WORKING ARRANGEMENT AND PROCEDURES

For the efficient and successful execution of this agreement , the PARTIES hereby agree to
undertake the following:

A. DUTIES AND RESPONSIBILITY OF Family Cernechez:

1. To advice recipient Basud Elementary School on the use of one apartment unit which will
serve as the official reading center.
2. No rental of place and no charge as to electricity and water bill.
3. Inform the school on when the project will start to observe safety precautions.

B. DUTIES AND RESPONSIBILITIES OF RECIPIENT BASUD ELEMENTARY SCHOOL

1. Inform the parents of the beneficiaries of the program.

2. Supervise the project and may suggest for a better output.
3. Maintain the cleanliness of the reading center.
4. Observe the IATF Protocol in the Reading Center.
 Article IV

EFFECTIVITY

This agreement shall be effective from the date of its signing and shall continue to be in force
until the completion of the project implementation.

Article V

SPECIAL PROVISIONS

All annexes and other documents attached to the memorandum of Agreement shall be
understood as forming part of this MOA. Provisions and contents thereof are therefore binding
on both parties unless part or whole documents are amended and mutually agreed upon by the
both parties.

IN WITNESS THEREOF, the PARTIES have hereto affixed their signature on the date and
place above-written.
