D. QUALITY OBSERVATIONS:

1. Random audit was conducted by ED to validate whether the cast parapet wall in GF has an approved WIR.
Both contractor, DBB and the design consultant, SSH cannot provide the approved WIR copy or even the
document/Aconex reference. Both parties state that approval is done on the day shift but cannot provide
an objective evidence to substantiate said claim. This may implicate any and/or all of the following;

a. Communication gap between the day and night shift key roles to handover essential and mandatory
quality control hold points or verification points is unrecognized due to lack of strict enforcement.
b. System restriction is a factor to be considered due to Aconex and WIR process workflow. This means that
the availability of approved WIR information may not be readily accessible to the night shift since the
final approving body which is LAD would take another shift for documentation and official transmittal of
the relevant WIR into ACONEX.

Conclusion:

Accomplishment of hold point as per ITP prior work execution by means of validation of the WIR approval by
LAD and the comments therein is imperative and mandatory to ensure the works done at site are as per the
contract requirements, therefore SSH and DBB must be able to verify and affirm the approval through
documentation without fail.

Recommendation:

Since the workflow constrains the validation through ACONEX due to lag time, the broadly contemporary
and practical solution is for the day shift team, DBB and SSH to fully coordinate with LAD (day shift) any time
before EOB matters relating to critical inspections which have work-fronts programmed by the evening of the
same day. LAD to ensure the status and comments of these inspections are provided and already written so
that a photo of the WIR front sheet with approval signature and comments by them (LAD) can be taken by
the DBB and SSH day shift team and handed over to the night shift for records and execution.

E. SAFETY OBSERVATIONS:

1. Combustible materials i.e. wood accumulation in Level 5 heightens fire load and fire risk

F. PROGRESS OBSERVATIONS:

1. Lack of qualified supervision and manpower:

It was observed that in GF, circa 16-20 cum of screed is already ready for placement. However, due to
insufficient manpower and supervision, DBB was unable to commence the said work front. A supervisor was
supposedly on duty but is otherwise due to sickness and no replacement cover was arranged.

Moreover, among the 9 masons in the roster, only 2 of whom are skilled in screed quality finish whereas the
rest are mason helpers. This does not only adversely affect efficient productivity but quality of the screed
finish as well especially when the light weight screed has fast-settling properties that demands immediate
finishing upon placement.

The contractor must be able to capitalize and maximise the night shift, accelerate the prerequisite works
which have linked logic to subsequent works and unlock critical work-fronts.

1. Traffic controllers and isolation measures are in place for moving vehicles and high-risk activities.
