1. SCOPE

1.1 This method statement is to outline and describe the activities of setting
up/dismantling Rest Shelter (20ft x 20ft), Portable Toilet, and storage area
(25m x 25m) on approved location by Petron.

2 OBJECTIVE

2.1 To provide Rest Shelter, Portable toilet and space for storage area during the
construction period.

3 RESOURCE ALLOCATION

3.1 Material:

3.1.1 Rest Shelter (20ft x 20ft) 1 set

3.1.2 Portable Toilet x 1 set

3.1.3 Storage (25m x 25m) two location

3.1.4 Drinking water

3.1.5 First Aid Kit

3.1.6 PTW Box

3.1.7 Wooden Table and chair

3.2 Manpower

3.2.1 Supervisor x 1 person

3.2.2 General Worker x 3 person

3.2.3 Backhoe Operator x 1 person

3.2.4 Lorry Operator x 2 person

4 WORK METHODOLOGY

4.1 Installation of Rest Shelter (20ft x 20ft), marking of storage area/pilling storage
area (25m x 25m), Portable Storage Cabin and Portable Toilet
-Location of the Rest Shelter shall be agreed by Petron before erecting of the
Shelter.
-Grading and leveling of Rest Shelter area
-Shelter shall be secure firmly to ground to avoid shelter being flown out.
-Once the shelter is erect, wooden table, Drinking water, First Aid Kit, PTW Box
are install.
-For material storage/piling storage area (25m x 25m) shall be agreed by Petron
and the location is graded and level for Construction Material Stockpile and
access entrance. Location of stockpile shall be marked and barricade/signage
to alert other Package Contractor the location will be utilized.
-For the Portable Toilet will provide Third Party Supplier which cleaning of
portable toilet will be done every weekly by Third Party Supplier. The location
for Portable Toiler shall be nearby to Rest Shelter area.
-For piling point area, access to the working platform shall be graded gentle
slope for enable pile rig to enter. Working platform area shall be graded to
close to balance level for enable piling rig to work. Steel Plate will be place on
top of the soil during the pile driving process.
- Refer to Attachment A for propose site set up plan

4.2 Vehicle that Mobilize Machinery/Material shall undergo on site pre-inspection by

Petron personal before escort into work site. Refer to Traffic control management
plan for maneuver of the heavy machinery into work site.

5 SAFETY PLAN

5.1 Daily toolbox shall be conducted.

5.2 Use of appropriate Personal Protective Equipment (PPE) such as coverall, Safety
Helmets, Safety Glass, Safety Shoes/Boots, etc relevant to the work being
carried.

5.3 All Instruments used must be inspected before operation.

5.4 Work permit shall be obtained before start the work

5.5 To ensure working on approved information.

5.6 Traffic Marshall shall be place during mobilization of heavy machinery/material

5.7 Daily Housekeeping
