抗精神病药物诱发代谢综合征的DNA甲基化研究进展 项思莹
综述
[ 摘要 ] 精神分裂症患者更易合并代谢综合征,其发生和发展可能与基因和环境相互作用有关。抗精神病药物是精神分裂症主要治疗
手段,会引起相关基因 DNA 甲基化的改变,进而影响药物疗效或产生代谢不良反应。该文就 DNA 甲基化与精神分裂症合并代谢综
合征之间的关系进行综述。

[Abstract] Schizophrenia patients are more likely to develop metabolic syndrome which is related to the interactions between gene and environment.
Antipsychotic drugs, the main treatments, can change the DNA methylation level of related genes and then influence the efficacy and adverse effects of
drugs. This article reviews the relationship between DNA methylation and schizophrenia with metabolic syndrome.

抗精神病药物是精神分裂症及其他伴有精神病性症状 病和 2 型糖尿病的患病风险,是导致精神障碍患者寿命缩
精神障碍的主要治疗手段,包括第一代(典型)和第二代 短的主要因素 [2]。
(非典型)药物。前者主要作用于中枢多巴胺 D2 受体,容 现代医学认为遗传因素、心理因素、环境因素是三大
易导致锥体外系不良反应。后者主要拮抗多巴胺和 5- 羟 重要致病因素,而环境及心理因素可通过表观遗传学修饰
色胺受体,对精神分裂症的阳性症状、阴性症状及认知症 改变相关基因的表达,参与疾病的发生与发展 [3-4]。表观遗

泛应用。精神分裂症患者需要长期服用抗精神病药物控制 非编码 RNA 等。其中,DNA 甲基化是真核生物中最稳定

过补充叶酸来改善精神分裂症患者的代谢综合征。 相关性,并未做更深入的机制探讨。
3.2 儿茶酚胺氧位甲基转移酶与代谢综合征
4 结语
儿 茶 酚 胺 氧 位 甲 基 转 移 酶(catechol-O-methyl
transferase,COMT)是一种儿茶酚胺降解酶,也参与叶酸 DNA 甲基化是高度动态化的过程,仅仅探讨特定样
代谢,协助 Met 转为 Hcy,引起 Hcy 含量升高 [31]
。COMT 本在特定状态下的甲基化差异是远远不够的。精神分裂症
常见突变为第 158 号的 Met 取代 Val。该酶活性在 Val/Val 人群相较于正常人群有特定基因的甲基化异常,抗精神病
型患者中比 Met/Met 型高 30% ~ 50% [32]
。有研究 [31]
证明 药物能靶向这些特定基因而起治疗作用。但是,抗精神病
