Pertamina : Mr. Londo, Mr. Jati, Mr. Irfan, Mr. Said.

As per ClasssNK Surveyor comment thru email on 12 April 2017 stated during ClassNK inspection found terrible
condition inside Double Bottom of H7009 that water and rubbish were remained inside Double Bottom caused
corrosion will be generated.
In respect to this issue, Yard QC propose an Action Plan duly reviewed by Project Manager and have been discussed
with Production Supervisor and Safety in charge. On this proposal, corrective action and preventive action
will be monitored by weekly basis with proper checklist and recorded for submission and approval by Project Manager.
Action Plan
1. Corrective Action:
1. Immediately clean all rubbish area affected inside Double Bottom.
2. Immediately do briefing to all workforce who have responsibilities to clean and maintain housekeeping
on their particular area by weekly basis.
Person in charge: All concern Supervisor.

2. Preventive Action:
1. Provide rubbish bin nearby working area each 10 m 2.
2. Provide signboard on designated rubbish bin area as collecting point of rubbish.
3. Do monitoring of housekeeping nearby working area & designated rubbish area by weekly basis.
4. Continuous record the checklist duly approved by Project Manager prior submission to ClassNK and Owner.
