SCRIPT: HEAD NURSE DE LIMA

Josha: Goodafternoon Nurses I’m Nurse De Lima you’re head nurse for this whole shift. To start with, I
already sent last night the schedule of your duty this week.

Diane: Yes Ma’am and nakapag endorse na ang outgoing nurses to us about sa charting and Doctor’s
order. There were some modifications lang po sa different cases ng patient but will try to solve problems
if things get worse especially to patient 302, tachycardic siya and upon endorsement, the outgoing nurse
administered vasodilator already.

Josha: Hm. Good, anyways Abubakar, Balbin and Balisi, you are assigned to room number 301-305 while
Amsid, Desiderio and Ahmad will be in room 306-309. Some Patients are stable and your task is to get
their v/s and aware naman kayo na please follow my instruction. Room 304 must administer pain
reliever and antibiotic PO at 10 AM.

Vince: Ma’am upon reading the chart of patient 304, suggest ko po na we will do catheterization na po sa
patient since may cellulitis po siya sa right leg, kasi if we stick to administer pain reliever everytime ang
pain will trigger an hour after she used the bathroom, baka mag-affect din ang ibang body due to that
medication and matanda na rin ang patient and may rheumatism. Catheterization to decrease the
chance of pain and relax ang patient.

Josha: Okay but, please follow my instruction kanina, I’ll be the one who’s in charge siya chart.

Zesh: Ma’am can you check my initial intervention to patient 305, almost febrile ang patient last 2 hours,
we can have towel bath for the patient to regulate the patient’s temperature since nakainom na si
patient ng paracetamol an hour ago, and medyo di daw siya makahinga upon endorsement ng nurse sa
akin, will it be possible to have chest x-ray already?

Josha: I’m too busy to check your notes, what is written sa chart and sa order ng patient, sundin Ninyo
and remember, I gave you all the instructions needed for this shift, there are no rooms for mistake dapat
uniform tayo. Okay the other three, you heard my instructions, right? Must avoid pressure ulcers to all
patients.

Elka: Yes Ma’am, but patient 307 has a pain on left rib cage, should we continue to turn the patient to
the left side considering the pain that the patient is experiencing? Should we perform heat compress?

Josha: Administer ibuprofen immediately and check if there’s improvement, don’t turn the patient until
mag affect na ang drug, then I will be the one to assess.

Eizannur: I have a question po Ma’am about sa chart po ni patient 308, for surgery siya later ng 2 PM, but
deviated ang v/s niya.

Josha: Okay, start checking the patient’s v/s again, then inform mo ako if stable or not ang v/s ng patient
before 2 PM.

Yunus: Ma’am kasi po umuubo na ang patient kaya I immediately administer acetylcysteine.

Jhada: Have you read the patient’s record, my history siya ng asthma, contraindicated sa kaniya ang
acetylcysteine, just prepare O2 sa bedside then check her O2 Sat and RR if di siya makahinga.

Kyla: Ma’am, upon checking kay patient 10, may excessive bleeding po siya sa lacerated area.

Jhada: Ms. Ersando I already told you to get gauze and apply pressure sa sugat, ako na ang mag susuture
ng sugat. How about the two Enriquez? Perform CPR sa patient alternate kayong dalawa.

Enriquez 2x: Yes po Ma’am

Banua: Ma’am, may excessive diarrhea po si patient 3 and sa lab result niya may fluid loss.

Jhada: Administer IV Dextrose Ms. Banua

Bello: I have a concern po Ma’am kay patient 5, di na makahinga ang patient.

Jhada: Administer 100 mg hydrocortisone heplock IV.

Carpio: Ma’am patient si patient 6, nakagat ng aso sa lower leg, nag dressing na ako ng sugat.

Jhada: Good, ano ba yan maging snappy naman kayo!
